The novel coronavirus pandemic is affecting the whole world and it is predicted that the process will continue for a long time. The health, economic and social problems caused by this process have greatly affected women as well. In addition to the problems endured by the general population, women are facing additional challenges involving their reproductive health, pregnancy, childbirth and postpartum period. Women are trying to deal with a range of concerns, from difficulties in accessing family planning services to being unable to meet their own basic reproductive health needs. Under the pandemic conditions, it has become increasingly difficult for social services to respond sensitively to women's particular needs. The needs of coronavirus patients take precedence, and the women’s needs remain in the background. Women are facing stress, anxiety, fear, as well as disease. So, midwifery practices need to be revised to adapt to the pandemic conditions. This revision must involve not only technical issues but also psychosocial adjustments. Women have been demanding home births on social media, and this is a subject that require some legal regulations. Respect for women's birth is one of their most fundamental rights. National criteria should be developed to accommodate the support structures needed for a home birth. The social support that midwives offer women during pregnancy, childbirth and the postpartum period, play a key role. Since it is important to increase the awareness of midwives in this regard, this article deals with women's needs for social support and possible midwifery approaches for supporting women through their pregnancy, childbirth, and the postpartum period.

Application of mixed hydrotropy in spectrophotometric analysis of frusemide in different formulations

The present study describes the use of an aqueous solution containing a blend of hydrotropic solubilizing agents (mixed hydrotropic substance’s solution) as a successful solvent system utilizing the concept of mixed hydrotropy for spectrophotometric analytical estimation of various conventional formulations as well as novel drug delivery systems. Frusemide, a poorly water-soluble drug, was estimated by application of mixed hydrotropic solubilization method. There was more than 15-fold enhancement in aqueous solubility of frusemide in a solution of blend of hydrotropic agents which consisted of 30% urea, 13.6% sodium acetate and 11.8% sodium citrate. This solvent mixture was employed to solubilize the drug from the fine powder of tablet formulations as well as the niosomes of frusemide. The selected λmax for spectrophotometric estimation was 333 nm. The hydrotropic agents used in the analysis and additives used in the manufacture of tablets and preparation of niosomes did not interfere in the analysis. Statistical data proved the accuracy, reproducibility and precision of the proposed method. The results suggested that proposed method is new, rapid, simple, accurate, and reproducible as well as employed aqueous solvent instead of organic solvents in estimation of drug from the dosage forms.

Evaluation and comparison of clinical efficacy of ambu auragain with i-gel, in patients undergoing laparoscopic cholecystectomy

Background and Aim: Second and third-generation supraglottic airway devices (SADs) are increasingly used in laparoscopic surgeries due to their improved oropharyngeal seal and gastric drainage. This study evaluated and compared the clinical efficacy of the Ambu AuraGain (AAU) and the i-gel in patients undergoing laparoscopic cholecystectomy. Materials and Methods: In this prospective, randomized comparative study, 80 adult patients (ASA I and II) were randomly assigned to two groups (n=40 each): Group A (Ambu AuraGain) and Group G (i-gel). The primary objective was to compare oropharyngeal leak pressure (OLP). Secondary objectives included insertion time, ease of insertion, success rate, fiberoptic view of the glottis, and postoperative complications. OLP and peak airway pressure (PAP) were measured at various intervals, including during carboperitoneum. Results: The mean OLP was significantly higher in Group A than in Group G, both after insertion (32.9 ± 3.45 vs. 26.53 ± 1.61 cm H2O; p<0.001) and after deflation of carboperitoneum (34.15 ±2.9 vs. 28.2 ±1.71 cm H2O; p<0.001). The margin of safety (OLP–PAP) was significantly higher in the AAU group at all times (p<0.001). However, the i-gel was significantly faster to insert (14.8 ±2.94 vs. 22 ± 3.74 seconds; p<0.001) and easier to place (100% easy vs. 37.5% easy; p<0.001). Fiberoptic alignment was significantly better in the i-gel group, with 70% achieving a Grade 4 view compared to 29% in the AAU group (p<0.001). Postoperative complications were low and comparable between groups. Conclusion: Ambu AuraGain provides a superior oropharyngeal seal and a higher safety margin against leaks during laparoscopic surgery compared to i-gel. Conversely, i-gel is superior in terms of ease of insertion, speed, and anatomical alignment with the glottis, making it a better conduit for fiberoptic-guided intubation. Assessment of drug prescribing pattern and prescription errors in elderly patients

The rate of aging is rapidly increasing and the term of geriatric refers to offer a medical care to elderly people. Drug consumption study on geriatric population is a vital issue since this group is totally neglected in Libya. The aim of this study was to assess drug utilization pattern among elderly patients in terms of world health organization core prescribing indicators and to evaluate the treatment cost. A total of 106 prescriptions were randomly collected from different community pharmacies located in Tripoli during the summer 2019, and were evaluated (total number of prescribed drugs is 359) for their clinical efficacy and safety. The findings revealed that 60 drugs acting on GIT (16.7%), 50 antimicrobial drugs (13.9%), 31 antihypertensive drugs (8.6%) and 30 drugs for respiratory diseases (8.4%) were prescribed to patients who are 65 years old or more. With regard to disease pattern in the elderly Libyan patients, 27 drugs were prescribed for diabetes mellitus (7.5%), 24 drugs for cardiovascular drugs (6.7%) and 14 drugs for anti-hyperlipidemia (3.9%). According to the WHO core drug use indicator pattern, the average number of drugs prescribed per encounter was 3.3. Prescription by generic name was low (41.5%). Antibiotics were 36.8% while injections were 26.6%. Thus, this study suggest that geriatric medicine and rational drug use should strongly be implanted in medical schools and teaching hospitals. Besides, a drug pattern use of the Libyan community is urgently required to avoid drug prescribing errors.

Analysis of few crude compounds and separation of leaf pigments in few medicinal plant species

For utilization of a certain biomass it is necessary to know its chemical composition. The present study is qualitative crude chemical analyses of nutritional value (crude compounds) such as acidity, starch, carohydrates, iron and calcium etc. from stem extracts of Tinospora cardifolia, Centella asiatica, Clerodendrum inerme and leaf extract of Ocimum sanctum, Lawsonia inermis and Piper nigrum were conducted and also separate the pigments by TLC methods. The results revealed that Tinospora cardifolia, biomolecules like starch and carbohydrates were present and elements like iron and calcium were absent. In Centella asiatica indicated the absence of biomolecules like starch and carbohydrates and only one molecules, i.e., calcium is present and iron is absent. Similarly, in Clerodendrum inerme, biomolecules like starch were absent and carbohydrates were present. Both the elements iron and calcium were absent in Clerodendrum plant extracts. In Ocimum sanctum, starch is absent carbohydrates are present, iron and calcium are absent. In Lawsonia inermis, the results are similar to Ocimum sp. i.e., only carbohydrates are present remaining starch, iron and calcium are absent in Ocimum leaf extract. Interestingly, in Piper nigrum, all chemical compounds are totally absent. Related to pigments analysis highest distance travelled pigments are carotenoids (4.3 cm) in O. sanctum and highest numbers of pigments were founded in C. inerme with six pigments and least number of pigments was recorded in O. sanctum (four) and L. inermis (four). Meanwhile, the highest Rf value recorded in O. sanctum and P. nigrum with 0.97 (chlorophyll a). Among the seven pigments in six species carotenoids, chlorophyll a and xanthophylls are common to all species but chlorophyll b, lutein, anthocyanin and lawsonin are rare to occurs in these six species for example Lawsonin occur only in P. nigrum.

Intraspecific population variability in goldstripe ponyfish, karalla daura sampled along the pakistan coast based on geomorphometric approach

The use of geo-morphometric variables to discriminate fish populations is one of the appropriate, effective and widely adopted approaches to study population dynamics. To determine intrinsic population variability in goldstripe ponyfish, Karalla daura sampled from Keti Bunder, Karachi Fish Harbour, Hawks Bay and Pasni along the coastal belt of Pakistan. Size frequency distribution based on normal distribution demonstrates that individuals of 13 cm total length were common in all groups. Some fifteen morphometric and six geometric variables were used to develop Principal Component Analysis (PCA) that revealed (76%, 13.9% and 9.8%) variation deduced from first three components. The population sampled from Karachi fish harbors showed significant taxonomic variability. An eigenvalue 7.251 and canonical correlation 0.937 among four groups was appraised from PCI whereas Wilks’ Lambda test of function λ = 0.27 was significant at P <0.05. The highest variance percentage was the most correlated among the groups defined by total length, girth, 2nd dorsal fin, pectoral fin rays, eye dia, snout to extended gill cover and head length parameters. PC-II accounted for 13.9% of the variance that are more correlated with standard length, snout to eye tip, eye tip to first dorsal fin, base of 1st dorsal fin, last fin ray to tip of caudal, anal fin ray to pelvic fin ray, chin to snout and pelvic fin to pectoral fin. Whereas PC-IV has 0.3 % variance denoted by total weight, tip caudal fin to last anal fin ray. The PCA displayed a clear discrimination by sampling site, especially specimen from Karachi fish harbor were distinct from other location. Keti Bandar specimen was not dispersing as much as by geographical discrimination. Overall, results revealed that different topographies and environmental conditions causing significant variation in populations of Karalla daura. Therefore, it is assumed that this species has ability to adopt different ecosystem for survival despite acquiring adverse impact on phenotypic characteristics.

Effect of dietary flavonoids on amine incorporation activity of transglutaminase 2 enzyme

Transglutaminase 2 (TG2) is an inducible transamidating acyltransferase that catalyzes Ca2+-dependent protein modifications. TG2 enzyme disruption has been implicated in several different disease processes and disorders such as Huntington and Parkinson’s diseases, cancers such as breast, ovarian and pancreatic cancers. Coeliac disease (CD) is the one disease state that TG2 activity plays a crucial role. To date, a gluten-free diet is the only accepted form of therapy for CD. Because of the important role of TG2 enzyme in the initiation of CD, therefore, this study was aimed at the identification of TG2 inhibitors from natural sources, as a potential intervention in CD therapy. Competitive amine inhibitors are the most widely used TG2 inhibitors because they are commercially available, chemically stable and relatively non-toxic in living systems. The natural products chosen for this study were dietary flavonoids. Flavonoids were extracted from different food samples. The flavonoids food extracts were subjected to the TG2 activity assays to examine their effect on the enzyme activity. The assays were carried out under optimal conditions of pH, Ca2+ and with N, N-dimethylcasein (acyl-donor) or casein (acyl-acceptor) as acyl-donor substrates and biotin cadaverine (acyl-acceptor) or TVQQEL as (acyl-donor) substrates. TG activity was measured by two different microplate assays, Biotin-labeled cadaverine incorporation assay and Biotin-labeled peptide cross-linking assay. In the TG2 amine incorporation activity, all of food extracts display a significant inhibition effect towards the human recombinant transglutaminase2 (hrTG2) and guinea pig transglutaminase 2 (gplTG2) (20 - 50% of inhibition). While in the TG2 cross-linking activity, the majority of food extracts displayed an inhibition effect on the gplTG2 cross-linking activity (50 - 70% of inhibition) but only the strawberry and kale extracts showed an effect on hrTG2 activity (40 - 50% of inhibition). The inhibition of TG2 activity can be considered as a potential therapeutic target in the treatment of CD.

Multimedia data encryption attempts to prevent unauthorized disclosure of confidential multimedia information in transit or storage. Security of multimedia files attracts more and more attention and many encryption methods have been proposed in literature. If we call a multimedia data stream (message) plaintext , the process of transforming the plaintext into unintelligible data stream is referred to as multimedia encryption (MME) where the encrypted message (data stream) is often named ciphertext. The process of transforming the ciphertext back into plaintext is termed decryption. We propose a new block cipher based on randomized key of size n × n where n is the block size and the block undergoes n 2 iterations with the plaintext. Every iteration generates the pseudo cipher text. The encryption process generate the ciphertext C with the help of the randomized key. The decryption apply the key in reverse order on the cipher text, to get back the plain text. This work deals with the problem of efficient multimedia data encryption.

Abstract The main objective of this paper is to examine the nexus between the reforms that have been carried out in Nigeria concerning the power sector, and the implications of the reforms on the transformation of agro-allied industries. The power sector, no doubt, can be said to have the potential that can propel national development considering the overall importance of energy to the survival of the nation in terms of job creation, industrial development and sustenance as well increase in revenue generation and Foreign Direct Investment opportunities, among others. The unfortunate situation, however, is that in spite of a number of policies enunciated by successive governments aimed at improving on the power sector, not much has been achieved. The multiplier effect of this kind of development has been seen in the widespread epileptic power shortages, downward spiral of efforts towards industrial development drive; unemployment, shortage of national revenue accruing to government coffers, bourgeoning and rising poverty levels, to mention but a few. All of these have further contributed to the growing army of problems currently being witnessed in the country at the moment. This study is a theoretical exposition of the existing works of scholars and experts in the broader field of power sector reforms. The paper is exploratory in structure and method of its investigation. The study concludes and recommends that in view of the prevailing socio-economic circumstances in Nigeria, genuine reforms in the energy sector devoid of political rhetorics will go a long way in mitigating some of the challenges of contemporary times in the power sector

Development and validation of rp-hplc method for simultaneous estimation of levocloperastine fendizoate and chlorpheniramine maleate in their combined dosage form

A simple and rapid RP-HPLC method has been developed and validated for the simultaneous determination of Levocloperastine fendizoate and Chlorpheniramine maleate syrup formulation. Resolution of the analytes was achieved within 10min, employing a mixture of 10mM mobile phase Buffer (pH 6.5): Acetonitrile (50:50, % v/v) as isocratic mobile phase, pumped at 1.0mL/ min through a C18 column (5μm particle size). The detection wavelength for the analytes was 227nm. The system suitability parameters were found to be acceptable. The linearity of response (r2>0.999) in the appropriate ranges (from 50% up to 150% of the expected concentrations of the analytes in the formulations), method accuracy (RSD<2.0%), repeatability and intermediate precision (RSD<2.0%), were confirmed. Robustness result indicates that the methods performance can withstand small variations in method parameters. Satisfactory results obtained in terms of analyte recovery and RSD, while analyzing marketed pharmaceutical preparations. Hence the method can be useful for regular analysis of this combination in marketed syrup formulation.

Dan-nucnet: a dual attention based framework for nuclei segmentation in cancer histology images under wild clinical conditions

Nuclei segmentation plays an essential role in histology analysis. The nuclei segmentation in histology images is challenging in variable conditions (clinical wild), such as poor staining quality, stain variability, tissue variability, and conditions having higher morphological variability. Recently, some deep learning models have been proposed for nuclei segmentation. However, these models rarely solve the problems mentioned above simultaneously. Most of the information in Hematoxylin and Eosin (H&E) stained histology images is in its channel, and the remaining information is in the spatial domain. We observed that most problems could be solved by considering channel and spatial features simultaneously, e.g., the spatial and channel features provide the solution to the morphological variability and staining variability, respectively. Therefore, we propose a novel spatial-channel attention-based modified UNet architecture with ResNet blocks in encoder layers. The UNet baseline preserves coarse and fine features, thus proving the solution to the tissue variability. The proposed method significantly improves the segmentation performance compared to the state-of-the-art methods on three different benchmark datasets. We demonstrate that the proposed model is generalized for 20 cancer sites, more than any reported literature. The proposed model is less complex than most state-of-the-art models. The impact of the proposed model is that it will help improve further procedures such as nuclei instance segmentation, nuclei classification, and cancer grading.
