Orodispersible and mucoadhesive buccal films: advances, formulation challenges, and future perspectives in oromucosal drug delivery

Mucosal systems such as orally disintegrating films (ODFs) and mucoadhesive buccal films (MBFs) have gained attention and become a potential alternative to the conventional oral drug delivery systems, especially for patients who are geriatric or pediatric or those having difficulty in swallowing or drugs with extensive first-pass metabolism. Such films provide benefits, including a quick onset of action, increased bioavailability, and patient compliance. Successful development entails meticulous optimization of ingredients such as film-forming polymers, plasticizers, surfactants, taste maskers, and material selection for the corresponding manufacturing process, including solvent casting, hot-melt extrusion, 3D printing, and electrospinning. Limitations remain, with challenges including low drug loading capacity, no unified evaluation standard, stability problems, and regulatory vagueness of the liposome carrier. Emerging trends signal the promise of nanocarrier-based films, solid dispersions, and stimuli-responsive systems to circumvent these barriers. Personalization technologies, specifically 3D printing and natural and biodegradable polymers, are expected to broaden the clinical use of oromucosal films. This article provides an overview of the current state of development, along with formulation and manufacturing considerations of ODFs and MBFs, the role of which is significant in addressing translational and regulatory hurdles.

Prevalence of cardiovascular risk factors in libyan patients with type 2 diabetes mellitus, 2013-2022

Cardiovascular disease (CVD) remains the leading reason of death among patients with diabetes mellitus (DM). Individuals with DM have a two to five time's greater risk of developing CVD than non-DM. Unfortunately, little data exists on Libya's prevalence and characteristics of cardiovascular risk factors. This study aimed to evaluate the cardiovascular risk factors and to assess their control among patients with type 2 DM (T2DM). All the patients with T2DM who attended the outpatient clinic at National Diabetes Hospital from September 2013 to April 2022 were interviewed and examined and included in this study. Demographical data age, smoking habit, body mass index measure, drug intake, history of previous cardiovascular events, blood pressure and fasting lipid profile were all recorded. 1049 patients have completed the study with a mean age of 54.06 ± 14.5 years old (females represented 68.0% of the studied sample size), there were different duration of diabetes between the studied sample, varying from newly diagnosed patients (n = 265, 25.2%) and above ten years of diabetes duration patients (n = 488, 46.5%). Active smoking in 129 patients (12.3%), the bodyweight of studied patients ranged from 44.0 Kg to 186 Kg (85.65 ± 1.73), 820 of the patients had uncontrolled body weight (78.85%). Uncontrolled blood pressure, even with treatment, was found in 304 patients (29.0%), uncontrolled dyslipidaemia in 45 patients (04.3%), and uncontrolled hyperglycaemia in 816 patients (77.9%). Established CVD was positive in 295 patients (28.2%), maximum age group was in 54 - 74 years (n = 516, 49.3%). The most typical age group with multiple (more than three) CVD risk factors was 54 - 74 years of age, were female patients with more than ten years of history of diabetes, uncontrolled hyperglycemia (HBA1c > 10%), uncontrolled body weight, uncontrolled blood pressure and dyslipidemia. Thus, the present data indicated that Libyan patients with diabetes mellitus have high CVD risk factors. Even in patients with previous events, these uncontrolled risk factors elevate the likelihood of repeated cardiovascular events. Patients with diabetes mellitus necessitate a more aggressive approach to control the modified risk factors such as hypertension, dyslipidemia, obesity and smoking.

An analytical overview of covid 2019- a scientific discussion, international journal of creative research thoughts

Corona virus pandemic has swept a major section of the world population. In terms of infectivity its higher than any pandemic in the recent past. Corona virus, a member of the family Coronaviridae, spread from bat to human through an intermediate host pangolin. It has lower mortality rate than the previous pandemics in last 100 years such as Spanish Flu(1918-20),Asian Flu (1957-58), SARS Corona virus (SARS-Cov) (2003), H1N1 Swine Flu Pandemic(2009-10), Middle East Respiratory Syndrome (MERS) (2012), West African Ebola (2014-16) etc. Research shows Covid spike attaches ACE2 receptor; it leads to a release of cytokine into the blood capillaries near the alveoli and may ultimately turn into pneumonia. The alveoli get collapsed leading ARDF. The pulmonary inflammation proceeds to systemic inflammation that causes multiple organ failure. Drug like Chloroquine, Lopinavir/ritonavir, and Remdesivir may halt the spread of the virus in the body. Antipyretic is prescribed to reduce the temperature. This article comprises of detail mechanism of the dreadful disease along with its diagnosis and proposed treatment. This exploratory study mainly designed to generate awareness and provide information to the individuals who want to know the A to Z of Covid in nutshell. This article may create a pathway to number of upcoming scientific researches and may attract them to work on this field to enhance the probability of inventing appropriate vaccine and medicine to fight against this contagious disease and to win the battle against this Global Pandemic.

Role of human liver microsomes in in vitro metabolism of metamizole

Metamizole or Novalgin® is a widely used well tolerated analgesic drug which is however compromised by agranulocytosis as adverse effect. Subsequent to nonenzymatic hydrolysis, the primary metabolic step is N-demethylation of 4-methylaminoantipyrine (4-MAA) to 4-aminoantipyrine (4-AA). The aim of the present study was to identify the human cytochrome P-450 enzyme (CYP) mediating this reaction. This study identified the relevant CYP using virus expressed isolated human CYP, human liver microsomes and rat liver microsomes with chemical inhibition studies. The substrate of 4-methylaminantipyrine was employed at six different concentrations (25, 50, 100, 400, 800 and 1200 µmol per l) with varying concentrations of selective inhibitors of CYP1A2 (furafylline, fluvoxamine), CYP3A4 (ketoconazole), CYP2A6 (coumarin), CYP2D6 (quinidine), CYP2C19 (omeprazole, fluvoxamine, tranylcypromine), CYP2C9 (sulphaphenazole) and CYP1A1 (alpha-naphthoflavone). 4-MAA and 4-AA were analyzed by HPLC and enzyme kinetic parameters (Km and Vmax) were determined by regression (Sigma plot 9.0). The N-demethylation of 4-MAA by microsomes prepared from baculovirus expressing human CYP was pronounced with CYP2C19. Intrinsic clearance of the most active enzymes were 0.092, 0.027 and 0.026 for the CYP enzymes 2C19, 2D6 and 1A2, respectively. Metabolism by human liver microsomes was strongly inhibited by fluvoxamin, omeprazole and tranylcypromine (IC50 of 0.07, 0.07 and 0.18, respectively) but with coumarin, sulphaphenazole, ketoconazole, moclobemid, quinidine alpha-naphthoflavone and furafylline were 0.79, 1.20, 1.36, 1.44, 3.46, 4.68 and 8.41, respectively. The enzyme CYP2C19 apparently has an important role in N-demethylation of 4-methylaminoantipyrine which should be further analyzed in clinical studies and which may also be interesting concerning the agranulocytosis.

Religion and ethical standards; the nexus in nigeria's democratic system and social -entrepreneurship perspectives

Nigeria is well known as a religious country where practically the citizens belong to one form of religion or the other unfortunately, the degrees of religious practces have not transformed into quality social, political, and economic lives for the people in terms of good governance, better standard of living, high ethical standards among others. It is descriptive research done as an exploratory paper in a mixed-method approach where some research questions were answered. Since ethical standards are predicated on simply knowing and doing what is good in doing to others what you will want done to you, people who claim to be religious as Nigerians should do better in this regard and many more given their faith. The effort to achieve this social rebirth can be achieved through social- entrepreneurship efforts this paper is advocating.

A zoonotic disease is an infectious disease caused by pathogens such as bacteria, virus, rickettsia, fungi, parasite or prion that has been transmitted from an animal, especially a vertebrate to human beings. Usually, the first infected human transmits the infectious agent to other humans very rapidly; it is one of the important concerns of zoonoses. The World Health Organization (WHO) currently list the Emerging infectious diseases (EID) of epidemic issues in their research and development blueprint as COVID-19 (Corona viruses), Middle East Respiratory Syndrome (MERS), Severe Acute Respiratory Syndrome (SARS), Ebola and Marburg viruses (Filovirus), Crimean Congo haemorrhagic fever (Nairo virus), Lassa Fever (Lassa virus), Nipah (Henipa virus ), Rift Valley fever (Phlebo virus) and Zika (Flavi virus). Due to the deadly pandemics facing by the globe, it is essential to understand the way of transmission of emerging viruses, its mode of infection and important measures to eliminate or control them from the world is essential. However, mankind overcomes so many deadly viruses with the help of scientific knowledge, through vaccination, new therapies, drugs etc. Nowadays, viral zoonoses like rabies (Lyssa virus) are 100% preventable through vaccination and medication even though, humans face lots of new threats especially emerging zoonosis diseases like Covid19. This review summarized the major zoonotic viruses affecting human beings, their source, pathogenicity and its important ways to overcome the transmission of microbes from animal to humankind.

In vitro antagonistic activity of some actinomycetes and fungal spp. isolated from rhizosphere of maize against fusarium sp. by bio-control dual inoculation method

Rhizosphere actinomycetes are promising biocontrol agents for use in agriculture and have been isolated from various plant species. In the present investigation, actinomycetes were isolated from rhizospheric soil of maize to test antagonistic activity against Fusarium verticillioides and fungal strain (T. viride) were procured from National fungal culture collection of India (NFCCI), T. harzianum and A. niger were isolated from soil to test their antagonistic effects against Fusarium verticillioides. In the present study bio-control dual inoculum method were followed to evaluate the bacterial and fungal strains against the pathogen and reveals that Single Bacterial Inoculum (SBI) of S. cinereus shows more effective in controlling the growth of Fusarium verticillioides (28 mm in treated plate) compare to control (45 mm fungal growth). But in Dual Bacterial Inoculum (DBI), the combination of S. albosporous and S. cinereus shows more effective (08 mm fungal growth in treated plate) compare to control (52 mm fungal growth). Similarly, in Single Fungal Inoculum (SFI), T. harzianum were more effective (18.8 mm fungal growth) and in Dual Fungal Inoculum (DFI), T. viridae and T. harzianum act as a more effective (2.5 mm fungal growth) against Fusarium verticillioides compare to controls (60 mm and 52 mm fungal growth respectively). Overall interpretation of the present work reveals that DBI and DFI were more effective than SBI and SFI against Fusarium verticillioides.

Cardiorenal, renocardiac, and reno-cardiocardiac syndromes: an updated review on general definitions, pathophysiology, and therapies (part 1)

Background: Acute and chronic heart or kidney failure affect each other in cardiorenal syndromes (CRS). In CRS, hemodynamic and non-hemodynamic changes occur, causing acute or progressive renal and cardiac failures. CRS is classified into five types based on the first organ failure and causes failure of the other organ. We believe that the current CRS classification is not the correct one that effectively describes the underlying cause of CRS. Hence, we consider it better to be classified into three categories (cardiorenal, renocardiac, and cardio-reno-cardiac syndrome) and then subdivided into acute and chronic types or types 1 and 2 (respectively, according to the onset of the underlying type of failure (i.e., acute or chronic). Other subtypes that occur inthe heart and dysfunction occur simultaneously are acute cardio-reno-cardiac syndrome (type 5) and Chronic cardio-reno-cardiac syndrome (type 6). Aim: In Part 1 of the review series, the pathophysiological mechanisms and clinical and therapeutic applications of all types of CRS will be narratively discussed and updated. Furthermore, we provide a comprehensive review of diagnostic biomarkers and their clinical significance in the identification, outcome prediction, and treatment of all CRS types. Method: An extensive search of PubMed, Google, EMBASE, Scopus, and Google Scholar was conducted for review articles, original articles, and commentaries published between Jan 2010 and Aug 2024 using different phrases, texts, and keywords, such as CRS, renocardiac syndrome, and CRS. The topics included secondary CRS, CRS pathogenesis, CRS therapy, SLGT inhibitor use in CRS, novel therapy in CRS types, and prevention of CRSs. Conclusion: Renal and cardiac failure in patients with CRS seem to have different pathophysiological mechanisms. Early detection and treatment can improve the outcomes of CRS. Clinical manifestations and therapy protocols vary according to pathophysiology. Hence, new guidelines and research on universal diagnostic and treatment techniques are urgently required. Moreover, the current nomenclature for CRS is confusing; therefore, we believe that a new nomenclature system should be introduced, reducing confusion and making differentiation between CRS types easier and less confusing.

An assessment of the quality of some portable water obtained within enugu metropolis

Provision of clean water is one of the United Nations Sustainable Development Goals. Water quality assessment as an ongoing exercise, in view of the threat of contamination from natural sources and human activities, plays a pivotal role in the sustainable management of water resources. Potable water samples were collected from Enugu Metropolis for water quality assessment. The physicochemical parameters were determined using APHA (1995 - 1998) protocols, and the presence of 7 heavy metals (Chromium, Zinc, Manganese, Silver, Cadmium, Iron and Lead) in the waters were determined using Atomic Absorption Spectrophotometer (AAS). The results of the physicochemical analysis of the four samples show a pH range of 8.60 - 8.95mg/l; this result showed that pH of the water samples were slightly alkaline and above the WHO stipulated range of 6.50 - 8.50. Acidity ranged from 5.00 - 6.88mg/l, while alkalinity values were between 7.52 - 13.00mg/l. The total dissolved solid (TDS) and total solids (TS) was present in very minute quantities and below their permissible limit of 0.45mg/l; while total suspended solid (TSS) was not present at all. The heavy metal analysis showed that all but 2 heavy metals (Cadmium and Lead) were below the permissible standard set by the WHO in all four samples. Cadmium was present in samples A and C with values 0.021mg/l and 0.006mg/l respectively, which is above its WHO limit of 0.005mg/l; while Lead was present in samples A, B and C with values of 0.021mg/l, 0.011mg/l and 0.015 mg/l respectively, which were above the WHO limit of 0.010mg/l. Given the well-documented toxicity of certain metals, there is a need for safety checks to be carried out in potable water companies to address the source of this contamination and ensure production of safer drinking water.

Library knowledge: the nigerian student perspective

This paper examines Nigerian students’ perception or approach to libraries. It cuts across the primary, secondary and tertiary institutions. The reason for the laxity in library real time usage is the primary driving force that led to the research. The research method adopted for this study was longitudinal method to get the right place of the actual issues without official cover-ups where necessary. Secondary data were also used to buttress the position of the research. The population approached for the longitudinal method was randomly picked at the different levels of schools. Parents were also in place at some point to drive home the true intent they have about their children and the use of libraries. Findings revealed that seeming laxity of both undergraduate and graduate students’ perception and attitude about the library is primarily because of the lacuna in their other levels of education before coming to the university. This served as a useful yard stick for the researchers developing workable recommendations that in their opinion will be of immense help to solving it. Index Terms— Library Knowledge, Nigerian Student, Primary School Library, Secondary School Library, Tertiary Library.

Kink and periodic solutions to the jimbo-miwa equation and the calogero-bogoyavlenskii-schiff equation

In this article, we form the exact wave solutions of the Jimbo-Miwa equation and the Calogero-Bogoyavlenskii-Schiff equation by applying the new generalized (G'/G)-expansion method. We explained the new generalized (G'/G)-expansion method to look for more general traveling wave solutions of the above mentioned equations. The traveling wave solutions attained by this method are in terms of hyperbolic, trigonometric and rational functions. The graphical representation of the obtained solutions is kink soliton, singular kink soliton, singular soliton and singular periodic solution. This method is very significant for extracting exact solutions of NLEEs which habitually occur in mathematical physics, engineering sciences and applied mathematics.

A study of wsn and analysis of packet drop during transmission

WSN is a low-power system and are often used in numerous monitoring uses, such as healthcare, environmental, and systemic health surveillance, in addition to military surveillance. It is important to reduce network resource usage since many of these applications need to be installed in locations that are virtually inaccessible to humans. Many protocols for WSN to extend the presence of the network have been established to solve this problem. In the energy efficiency of WSN networks, routing protocols play an important role since they help minimize power usage and response time and provide sensor networks with high data density and service quality. This study also employed a Hopfield neural network and the findings from this study are presented next to each other to enable comparison. This paper also discusses how to easily and accurately capture and handle WSN collisions. Future experiments that require the usage of neural networks and so many fuzzy structures will be able to prevent a crash in these respects.

Pathfinder core rulebook pdf - pathfinder 2e

While playing the Pathfinder RPG, the Game Master describes the events that occur in the game world, and the players take turns describing what their characters do in response to those events. Unlike storytelling, however, the actions of the players and the characters controlled by the Game Master (frequently called non-player characters, or NPCs) are not certain. Most actions require dice rolls to determine success, with some tasks being more difficult than others. Each character is better at some things than he is at other things, granting him bonuses based on his skills and abilities. Whenever a roll is required, the roll is noted as “d#,” with the “#” representing the number of sides on the die. If you need to roll multiple dice of the same type, there will be a number before the “d.” For example, if you are required to roll 4d6, you should roll four six-sided dice and add the results together. Sometimes there will be a + or – after the notation, meaning that you add that number to, or subtract it from, the total results of the dice (not to each individual die rolled). Most die rolls in the game use a d20 with a number of modifiers based on the character's skills, his or her abilities, and the situation. Generally speaking, rolling high is better than rolling low. Percentile rolls are a special case, indicated as rolling d%. You can generate a random number in this range by rolling two differently colored ten-sided dice (2d10). Pick one color to represent the tens digit, then roll both dice. If the die chosen to be the tens digit rolls a “4” and the other d10 rolls a “2,” then you've generated a 42. A zero on the tens digit die indicates a result from 1 to 9, or 100 if both dice result in a zero. Some d10s are printed with “10,” “20,” “30,” and so on in order to make reading d% rolls easier. Unless otherwise noted, whenever you must round a number, always round down. As your character goes on adventures, he earns gold, magic items, and experience points. Gold can be used to purchase better equipment, while magic items possess powerful abilities that enhance your character. Experience points are awarded for overcoming challenges and completing major storylines. When your character has earned enough experience points, he increases his character level by one, granting him new powers and abilities that allow him to take on even greater challenges.
