Controlling the apex in early onset scoliosis through active apex correction (apc) non fusion growth modulating technique, is it a myth?

Study DesignA multicenter retrospective study. ObjectivesTo determine the rate of Apex facet fusion in children with Early Onset Scoliosis treated surgically with the Active Apex Correction (APC) technique. MethodsSeventeen patients were treated with the APC technique as index surgery for Early Onset Scoliosis with more than 1 year of follow-up. A 3D CT scan was done to determine the facet fusion rate in the deformity’s apex controlled with posterior tethering. ResultsThe average follow-up time was 26.4 months (12-56), Average age at index surgery was 81.2 months (30-132), and average number of surgeries done 1.3. Apical vertebrae studied for facet fusion were the 3 vertebrae in the apex in each patient that were subjected to posterior tethering according to the APC technique. In total they were 86 apical vertebrae (172 Facets studied with 86 convex side, 86 concave side). Our observations showed that 29 facet joints were fused (16% of the total facets studied),15 were on the convex, 14 on the concave side (no statistically significant difference). Regarding the facet joint distance in the non-fused facets was 0.99 mm on the convex side and 1.08 mm on the concave side with no statistical significance difference. ConclusionAPC for Early Onset Scoliosis achieves apical growth modulation and control utilizing posterior tethering without inducing fusion. This study demonstrated that APC is an effective non-fusion technique through the low incidence of facet fusion levels at the Apex, limiting the crankshaft phenomena seen in cases with apex control through arthrodesis.

On the impact of debris accumulation on power production of marine hydrokinetic turbines: insights gained via les

We present a series of large-eddy simulations to systematically investigate the impact of debris accumulation on the hydrodynamics and power production of a utility-scale marine hydrokinetic (MHK) turbine under various debris loads lodged on the upstream face of the turbine tower. The turbine blades are modeled using turbine resolving, actuator line, and actuator surface methods. Moreover, the influence of debris on the flow field is captured by directly resolving individual logs and employing a novel debris model. Analyzing the hydrodynamics effects of various debris accumulations, we show that an increase in the density of debris accumulation leads to more flow bypassing beneath the turbine blade. This, in turn, reduces the flow momentum that reaches the MHK blades at the lower depths, inducing significant fluctuation in power production. Further, it is shown that debris-induced turbulent fluctuations contribute to significant variability in the MHK turbine’s power production.

The rate of oxygen uptake in male was higher tha control animal. Rate of oxygen consumption showed a significant decrease in both sexes with increase in duration of exposure to fluoride.

Molecular characterization of melanin pigment producing actinomycetes.

The soil samples were collected from Melia dubia plantations in two different locations in Tamil Nadu. A total of twenty five actinomycetes isolates were isolated and they were screened for melanin pigment production using different culture media. Only two isolates were able to produce melanin pigment under in vitro study and one isolate showed positive reaction to L-tyrosine substrate. Based on morphological and bio-chemical characters the isolate was identified as Streptomyces sp. The DNA of the isolate was isolated and its 16S rDNA gene was amplified and sequenced. The phylogenetic analysis of Streptomyces puniciscabiei was carried out.

Pelvic inϑlammatory disease (Waram al-Rahim)is one of the most serious gynaecological infections of the upper female genital tract with the worldwide annual rate of 10-20/1000 women of reproductive age. The objective planned for the study was to evaluate the effect of Sharbat-e-Deenar in uncomplicated Pelvic Inϑlammatory Disease (uPID). A pre and post observational single group clinical study was carried out in the Department of IlmulQabalatwaAmrazeNiswan, National Institute of Unani Medicine, Hospital, Bengaluru. Diagnosed cases (n=30) of uPID were included in the study. Patients with complicated PID, history of antibiotic therapy within seven days of recruitment; delivery, abortion or gynecologic surgery within the last 30 days, systemic diseases, malignancies, pregnancy & lactation were excluded. Sharbate-Deenarconsists of Poste baikhekasniandTukhmekasni(Cichorium intybus Linn.), Gulesurkh (Rosa damascene mill.), Tukhmekasoos(Cuscutare৒lexaRoxb.) and Rewandchini (Rheum emodiWall. ex Meissn) were administered orally in a dose of 20 ml twice daily for 14 days. Outcome measures were 30-70% clinical improvement in Visual Analogue Scale (VAS) score for lower abdominal pain and McCormack Pain Scale (McPS) score for abdominopelvic tenderness and White Blood Cells (WBCs) count <10 on saline microscopy of discharge. Data were analyzed using paired Student ‘t’ test, Wilcoxon Signed rank test and Paired Proportion test.Clinical improvement of 30-70% in McPS and VAS score was achieved in 96.7% (p<0.001**) and 93.3% (p<0.001**) patients respectively. WBCs count <10 on saline microscopy of discharge was achieved in 70% patients (p<0.001**).Sharbat-e-Deenarwas effective in improving the sign & symptoms of PID due to its anti-microbial, anti-inϑlammatory, antispasmodic and anti-oxidant properties. No adverse effect of the research drug was encountered during the study. Hence, Sharbat-e-Deenar serves as an effective alternative in patients with PID, proving the research hypothesis.

The problem studied in this research is the Implementation of the Student Team Achievement Divisions Strategy in integrated thematic learning at SD Darus Sholah, Jember. This research approach uses a qualitative approach with the type of phenomenological research. Methods of data collection using interviews, observation and documentation. Data analysis used the interactive model of Miles and Huberman, with the process of data collection, condensation, data display, and data verification. To test the validity of the data using triangulation. The results of this study indicate that: 1) At the presentation stage, the teacher starts with story material, reviews previous material, and learning activities in teams. 2) The teacher prepares a worksheet as a guide for group work and each member can contribute, the teacher makes observations, provides guidance, motivation, and assistance if needed. 3) Individual tests in groups. 4) Score development for individuals, the teacher combines the previous score with the final score. 5) Appreciation for the team is based on the assessment of individuals in the group so that the process of group assessment recapitulation is based on individual assessment of each group. Sorting of human mesenchymal stem cells by applying optimally designed microfluidic chip filtration

Human bone marrow-derived mesenchymal stem cells (hMSCs) consist of heterogeneous subpopulations with different multipotent properties: small and large cells with high and low multipotency, respectively. Accordingly, sorting out a target subpopulation from the others is very important to increase the effectiveness of cell-based therapy. We performed flow-based sorting of hMSCs by using optimally designed microfluidic chips based on the hydrodynamic filtration (HDF) principle. The chip was designed with the parameters rigorously determined by the complete analysis of laminar flow for flow fraction and complicated networks of main and multi-branched channels for hMSCs sorting into three subpopulations: small (<25>40 μm) cells. By focusing with a proper ratio between main and side flows, cells migrate toward the sidewall due to a virtual boundary of fluid layers and enter the branch channels. This opens the possibility of sorting stem cells rapidly without damage. Over 86% recovery was achieved for each population of cells with complete purity in small cells, but the sorting efficiency of cells is slightly lower than that of rigid model particles, due to the effect of cell deformation. Finally, we confirmed that our method could successfully fractionate the three subpopulations of hMSCs by analyzing the surface marker expressions of cells from each outlet.

Morphological and chemical traits associated with resistance against spotted pod borer, maruca vitrata in pigeonpea

Host plant resistance is an important tool for minimizing the losses in pigeonpea due to spotted pod borer, Maruca vitrata, which is the most threatening hidden pest of pigeonpea crop. Experiment was carried out to study the morphological and chemical traits in pigeonpea associated with expression of resistance to M. vitrata and their incidence in different sowing dates. The incidence of M. vitrata varied significantly among different sowing dates. Crop sown on D2 (1st week of July) resulted in maximum pod infestation by M. vitrata (13.1%). Minimum pod infestation (2.3%) and (2.9%) was recorded in D4 (3rd week of July) and D1 (3rd week of June) sown crop, respectively. Among different varieties, AL-201 registered lowest pod infestation (4.3%) as compared to Pusa-992 (7.9%). The non-glandular (type A), and glandular (type B) pod trichome density of top, middle and lower canopy of the plant and the pod wall thickness was responsible for the resistance to M. vitrata. Whereas, non-glandular (type C) pod trichomes, pod length, seed width and number of seeds per pod were associated with the susceptibility to spotted pod borer. Expression of resistance to M. vitrata pod infestation was associated with low amounts of crude protein and total soluble sugar and higher amount of fat content, phenol content and tannin content of seed as well as pod wall

Acute gallbladder disease – the virtue of performing a laparoscopic cholecystectomy in the early stages of case management

Background: In a Benghazi medical center, this observational study assessed the role of laparoscopic cholecystectomy (LC) in the early stages of acute gallbladder illness. Materials and Methods: From January 2022 to December 2022, all emergency and urgent admissions for acute gallbladder disease were identified, and additionally, data from hospital medical records were then evaluated retrospectively. Results: Out of a total of 334 patients with gallstone diseases, we found 321 individuals (206 with acute biliary colic, 115 with acute calculous cholecystitis), and 13 with acalculous cholecystitis. Early laparoscopic surgery for acute calculous cholecystitis had a greater conversion rate than procedures for acute biliary discomfort (19% vs. 4%; p=0.002). When surgery was performed 72 h from the onset of the symptoms for patients with acute calculous cholecystitis, the conversion rate was considerably lower (one out of 25) than when surgery was postponed past 72 h (14 out of 53) or later performed electively (seven out of 23) (p=0.014). In comparison to elective surgery for acute biliary colic (three of 66), elective surgery for prior acute cholecystitis was linked to a higher conversion rate (seven of 22 patients) (p=0.002). Conclusion: LC for acute calculous cholecystitis needs to be done within 72 h of the onset of symptoms, and it is worthwhile.

Online medical education in india – different challenges and probable solutions in the age of covid-19

Introduction: During the COVID-19 pandemic, most educational institutions have opted for online education rather than traditional modes of education to protect their employees and students. Online education has been gaining momentum in almost all countries around the world. This coincides with the recently introduced competency-based medical education in India which has embraced online education. This poses a new challenge for the institutions involved, the instructors or teachers, and the students since they must adapt quickly to the new mode of learning. Online education requires teachers to improve their competency in three major areas; pedagogy, technology, and content knowledge. Some of the challenges include; lack of technological skill, poor time management and lack of infrastructure. As technology rapidly advances, health care education systems must also advance in tandem. To implement the new competency-based system and online education, the institutions and the individuals must realize the importance of online education, identify the barriers and quickly work on solutions for success. Methods: This review was conducted based on various research papers on the topic of online medical education, the challenges faced by faculty members, and the opinion of students on this dilemma. Search terms included online medical education, COVID19, competency-based medical education. Conclusion: This review identified various challenges posed by online education on the current medical curriculum, faced by both faculty members and students, especially under the light of the Competency-Based Undergraduate Curriculum for Indian Graduates. Different solutions were proposed to overcome these challenges.

An audit on the amount of drug wastage and the cost related to the disposal of unused intravenous agents in the operating theatres in a tertiary care hospital

Background: This manuscript presents an audit focused on assessing the extent of drug wastage and associated costs linked to the disposal of unused intravenous agents within the operating theatres of a tertiary care hospital. The study aims to carefully look at drug wastage, figure out why it happens, and propose strategies for optimizing resource utilization and cost-efficiency in clinical settings. Materials and Methods: This audit is a prospective observational study carried out in a tertiary care hospital. The amount of drug loaded preoperatively, the amount of drug utilized, the amount of drug unutilized and discarded, the total quantity of drug wasted, and the cost related to the wastage were calculated in percentage. Results: The maximum wastage of loaded drugs was seen with atropine (100%), followed by ephedrine (93.6%), propofol (52.5%), phenylephrine (35.2%), atracurium (28.6%), dexmedetomidine (27.8%), fentanyl (16.8%), vecuronium (16.2%) and morphine (6.4%). The cost analysis revealed that 36.3% of the total loaded drugs were wasted amounting to Rs.46903.54. The cost of wastage of propofol was maximum with Rs.14006 which is 29.8% of total cost wastage followed by atracurium 21%(Rs.9856), dexmedetomidine 18.4%(Rs.8687.5), ephedrine 14.7%(Rs.6919), phenylephrine 10.4%(Rs.4910), fentanyl 3.7%(Rs.1780), atropine 1.9% (Rs.906), vecuronium 1.2% (Rs.563.76), Morphine 0.18% (Rs.85.28). Conclusion: In our audit, the maximum drug wastage was observed with atropine (100%) amounting to Rs.906, and the cost of wastage was maximum with propofol amounting to Rs.14006 which was 29.8% of the total cost of wastage. Our audit underscores the importance of proactive management of drug wastage and disposal costs in healthcare settings, particularly within operating theatres where intravenous agents play a crucial role in patient treatment. By conducting a comprehensive audit and proposing targeted interventions, healthcare institutions can optimize resource utilization, enhance sustainability, and ultimately improve patient outcomes.

Beliefs and practices in taking licensure examination for teachers as spectrum of teachers’ moral principles: a case probe

The study employed Appreciative Inquiry framework to have an in-depth investigation in the personal lives of the participants and to draw connections between their beliefs and practices in taking Licensure Examination for Teachers and how these superstitions reflect and affect their moral principles. Specifically, it aimed to: (1) determine the superstitious beliefs followed by the examinees before, during, and after taking the Licensure Examination for Teachers (LET); (2) determine the positive and negative impact of these superstitions in the aspect of taking LET; and, (3) identify the effects of these superstitions in the characters and principles of the teachers. Semi-structured interview and focus group discussion were used and the data-sources were triangulated to come-up with a stronger analysis. Audit trail and member checking were followed to ensure that there were no biases in the interpretation of the data and to establish the validity of the results. Findings revealed that the student examinees followed different superstitious beliefs before, during and after the exam which have positive and negative effects. The motive of avoiding failure in the examination predominates over the motive of achieving success. The positive and negative effects of superstitions vary from one person's experience to another. Moreover, the assertion and personal disposition to pass the exam became the product of student examinees' projection. The superstitious notions were initiated to discard the student examinees' fears and to safeguard their emotional sphere against negative feelings and any untoward possibilities. Meanwhile, in the development of moral principle, the student examinees' family, peers and the society are important factors in the establishment of their belief system which later on reflects the moral principles they uphold.

Uv spectrophotometric determination of cefixime in bulk and its dosage form

A novel, simple, accurate, sensitive, reproducible, economical and less time consuming spectroscopic method was developed and validated for determination of cefixime. The solvent used was 0.1 N HCL and the absorbance maxima or the λmax was found to be 283.0 nm and 303nm for zero order and first order derivative respectively. This method obeyes Beer’s Law for the concentration range of 8–16 µg/ml for cefixime. The proposed method was been validated statistically as per the ICH guidelines for linearity, accuracy, precision, specificity, LOD and LOQ. The method developed and validated successfully for the quantitative analysis of cefixime in bulk and dosage form.

Early diagnosis of tuberculosis using deep learning approach for iot based healthcare applications

In the modern world, Tuberculosis (TB) is regarded as a serious health issue with a high rate of mortality. TB can be cured completely by early diagnosis. For achieving this, one tool utilized is CXR (Chest X-rays) which is used to screen active TB. An enhanced deep learning (DL) model is implemented for automatic Tuberculosis detection. This work undergoes the phases like preprocessing, segmentation, feature extraction, and optimized classification. Initially, the CXR image is preprocessed and segmented using AFCM (Adaptive Fuzzy C means) clustering. Then, feature extraction and several features are extracted. Finally, these features are given to the DL classifier Deep Belief Network (DBN). To improve the classification accuracy and to optimize the DBN, a metaheuristic optimization Adaptive Monarch butterfly optimization (AMBO) algorithm is used. Here, the Deep Belief Network with Adaptive Monarch butterfly optimization (DBN-AMBO) is used for enhancing the accuracy, reducing the error function, and optimizing weighting parameters. The overall implementation is carried out on the Python platform. The overall performance evaluations of the DBN-AMBO were carried out on MC and SC datasets and compared over the other approaches on the basis of certain metrics.

Classifying jordanian hotels based on their tqm implementations

This study aims at classifing hotels in Jordan into groups based on their Total Quality Management (TQM) implementation. Using a survey methodology, the TQM questionnaire was designed to measure the level of TQM implementation throughout Critical Success Factors (CSFs) which are necessary for TQM implementation. A total of 345 TQM questionnaires were distributed to managers, working in 17 four- and five-star Jordanian hotels. The researcher obtained 227 usable TQM questionnaires. The results classified Jordanian hotels based on the CSFs for TQM implementation. More specifically, using cluster analysis on the CSFs of TQM resulted in two groups of hotels: ‘low TQM adopters’ and ‘high TQM adopters’. These two groups showed significant differences across the TQM CSFs
