Ayurvedic lipid based rasayans- a perspective on the preparation and pharmacological significance of lipids on the bioavailability of phytoconstituents

For thousands of years, lipid based Ayurvedic formulations have been made in India, and the craft has survived down the millennia up to the present time. Some of these deliciously potent phytonutrient preparations are very popular and have sustained the test of time pertaining to their efficacy. Recent researches on the role of phytonutrients in promoting cardio-pulmonary, brain and immune health substantially buttress the philosophy underlying the use of lipids in preparing these emulsions, since a large number of these bioactives are lipophilic. Being lipoidic, they are absorbed through the lacteals in the small intestine, and are then transported through the thoracic duct directly to the heart, bypassing the liver. The formulations utilizing ghee (clarified butter) or sesame oil as the carrier lipid, either while frying the myrobalams or as Anupana (adjuvant), have special significance in modulating bodily immunity, since the immune system is housed in lymphatics which are lipid rich. Amla and lipid based Ayurvedic rasayans (rejuvenating formulations) are a popular and highly palatable group of phytonutraceutical preparations. This group of polyherbal adaptogenic formulations is classified separately from other formulations in Ayurvedic therapeutics. Several of these healthpromoting rasayans are suitable to be consumed by all age-groups in the recommended season and dose. Current research on endothelial and immune cell receptor mediated uptake of lipoidic molecules, together with the knowledge of lipid absorption pathways, lends credence to the usefulness of rasayans in targeting the cardio-pulmonary and immune systems. An attempt has been made in this paper to elucidate the mechanisms underpinning the complex interplay between lipid delivered hydrophobic phyto-molecules, systemic lymphatics and the Immune system.

Thus, it is important for local workers to understand the leadership style of expatriates from the companies’ leaders and managers. The purpose of this research is to see the effect of transformational leadership style on job satisfaction conducted at PT Musim Mas, Medan. This research uses quantitative method with simple random sampling of 36 employees working in the company. The test results show that transformational leadership style has a positive significant effect on job satisfaction at PT Musim Mas, Medan as much as 69.5%.

Off-label use of misoprostol in obstetric-gynecology patients at a private hospital in banyumas

Using off-label drugs in obstetric-gynecology patients requires caution because of the high risk of pregnancy. Although there is still debate, data profiling its use in clinical practice is lacking. This study aims to observe the use of off-label drugs in obstetrics and gynecology patients in private hospitals, especially the pill Misoprostol. This study used a descriptive observational approach to hospital medical record data. Retrieval of research data retrospectively at Bunda General Hospital (X Hospital) for July 2017 – December 2017 and Sinar Kasih Hospital (Y Hospital) for January 2018 – December 2018. The number of samples was determined using the Slovin formula, with a systematic random sampling technique at X Hospital and total sampling at Y Hospital. The samples were 220 RM at X 130 Hospital and Y 90 Hospital. The research was conducted in the period January 2019 - March 2019. The results were analyzed univariately using several related references. The results showed 26.92% of misoprostol off-label use in X Hospital and 4% in Y Hospital. The off-label drug found was Misoprostol which belongs to the category of off-label indications (Missed Abortion, Incomplete Abortion, Blighted Ovum, and Induction of labor in cases of Serotinus, KPD, and IUFD), off-label doses (50 µg, 100 µg, and 400 µg), and off-label routes of administration (sublingual and vaginal). ​

The reliability and validity of the vietnamese translation of same-sex marriage scale

Vietnam, in many ways, remains a Confucian nation. The Confucian heritage touches many aspects of people’s lives. In this context, homosexuals do not face severe punishments, but face social stigma or public misunderstanding. Same-Sex Marriage is also not accepted because of limited legal support. Therefore, there is a need to study attitudes towards same-sex marriage in Vietnam. The purpose of this article was to validate a Vietnamses version of the Attitudes Toward Same-Sex Marriage Scale (ATSM) designed by Pearl & Galupo (2007) in a Vietnamese population. The sample in this study was 570 students from six universities in Ho Chi Minh City, Vietnam. The data collection method used was face-to-face questionnaire. Students completed the Attitudes Toward Same-Sex Marriage Scale and other variables in the questionnaire. The respondent’s characteristics also explored through the survey include: gender, what year students are from, religion of the students, what region students are from and their ethnicity. The study found the Vietnamese translation of the ATSM scale both reliable and valid and suitable for research use.

The 2019 coronavirus (COVID-19) pandemic is spreading worldwide, with a spectacular increase in death missing any effective therapeutic treatment up to now. Molecular docking is a recognized computational tool to assist in early drug discovery and development. Molecular docking analysis was carried out using 2-(4-(aminomethyl)phenyl)isoindoline-1,3-dione Schiff base conjugates with SARS-CoV-2 protease enzyme and COVID-19 main protease in apo form 6M03). The compounds with the best normalized docking scores to protease enzyme (6LU7) were ARG3 (-8.1 kcal/mole), ARG7 (-8.1 kcal/mole) and ARG6 (-8.0 kcal/mole). The best docking ligands for main protease in apo form (6M03) were ARG7 (-8.7 kcal/mole), ARG6 (-8.6 kcal/mole) and ARG3 (-8.4 kcal/mole). The structural similarity between these conjugates inspired us to perform in silico studies to check their possible binding interactions with essential SARS-CoV-2 proteins. These studies provide insight into the potential binding between Schiff base derivatives and SARS-CoV-2 proteins to provide an insight for finding an effective therapy. Finally, ADMET calculations were performed for the Schiff base compounds to predict their pharmacokinetic profiles.

Primary intraosseous xanthogranuloma in adult cervical spine: a case report of benign cause of lytic bone lesion

Lytic lesions in adult spine are a common manifestation of aggressive disease such as primary bone tumor, metastasis, myeloma, or infectious pathology. Xanthoma arising in the spine with purely intraosseous component is an extremely rare occurrence with only six cases reported in the adult population, none in the cervical region. We report the first case of primary xanthoma of the cervical spine in a 50‑year‑old male solely confined to osseous compartment. The imaging mimics of lytic lesion with expansile mass in adult spine are reiterated.

Evaluating the effect of covid-19 on community pharmacist’s practice

Pharmacists stand alongside with other professionals in representing essential healthcare during health emergencies such as COVID-19 pandemic. The present study seeks to explore the behavior and attitude of community pharmacists across various pharmacies throughout this pandemic towards the safety of workplace environment for staff and patients. An online questionnaire has been prepared and distributed to 145 community pharmacies during March to May, 2020. The collected results and data showed that community pharmacists in Libya have a good knowledge and adherence to preventive and protective measures published by FIP, WHO and other health-related organizations to protect themselves and society from infection. The pharmacist role in educating and information provision of the current disease and its management is still going on regardless the pandemic. A negative evaluation to the government support during the pandemic is clear from participant's replies. In Conclusion, this study looks forward to disclosing the current activities undertaken across various community pharmacy settings concerning safety of the workplace environment for both staff and patients. A clear and relatively realistic picture of the extent of commitment and knowledge of pharmacists of the necessary aspects needed to deal with this pandemic within community pharmacies is suggested. The extent of community pharmacist's knowledge, readiness and speed of their response to such a pandemic have been identified.

A scoping review of changes to patient-doctor communication during covid-19

Effective communication between patients and doctors is fundamental to high-quality healthcare, patient safety, and overall satisfaction. However, the onset of COVID-19 has prompted significant shifts in communication from in-room and face-to-face interactions to virtual consults. The impact of this pandemic related change on patient-doctor communication goals, processes, attributes, and environment remains unclear. We undertook a scoping review involving the systematic search of seven academic databases for relevant articles published up to and including June 2021. In total, 47 articles were identified that met the inclusion criteria. We applied the patient-doctor communication framework to guide our deductive thematic analysis of articles included, sorting results from reported studies and position papers into themes and sub-themes. The theme of communication goals highlighted sub-themes related to patient safety, convenience, affordability, and satisfaction; preparation included sub-themes on technology interventions, workforce training, and digital literacy; participant attributes included compassion for doctors and rebuilding trust among patients; and communication process included issues related to telemedicine or video conferencing, challenges with diminished patient privacy, and distractions in the patient's home setting. Finally, the environment theme included insights into doctors' workload, isolation, and anxiety and how changes requiring increases in virtual consults iteratively altered confidence in care provision and communication with patients. Results of the scoping review provide important insights for strengthening virtual patient-doctor interactions, including target areas for training and professional development during and beyond the current pandemic.

In-silico drug design, synthesis, characterization and biological evaluation of 2-azetidinone derivatives for antileukemic activity

2-Azetidinone shows biological activities like anti-bacterial, anti-microbial activity, anti-tubercular activity, and anti-cancer activity. 2-azetdinone derivatives were synthesized by simple procedures. The first step is synthesis of benzohydrazide through nucleophilic substitution reaction between methyl benzoate and hydrazine hydrate. The above formed compound is then treated with substituted aromatic aldehydes in the presence of catalytic amount of concentrated hydrochloric acid with stirring for one hour to give benzohydrazone which results in the formation of Schiff bases.Schiff bases undergone cyclisation in the presence of chloroacetylchloride and diethylenediamine by using ethanol as a solvent upon stirring for 4 hour’s yielded 2-azetidinone derivatives. The in-silico anti-leukemic activity was determined by using the computational tools i.e. “PASS Online”, “AutoDock4.2” and “ADMET” properties by online software’s. Among these six derivatives compounds (AZT-6) was shown more activity when compared with the other five compounds.

Analgesic and anti-inflammatory potential of four varieties of bell pepper (capsicum annum l.) in rodents

The aim of the study was to evaluate the analgesic and anti-inflammatory activity of four different colored (green, yellow, orange, and red) sweet bell peppers (Capsicum annuum L.) are available at the local market in Karachi, Pakistan. Their 95% ethanol extracts at 200 and 400 mg/kg were prepared and compared with commonly used analgesics (aspirin) and anti-inflammatory agents supporting its traditional use. The analgesic effects of 95% ethanol extracts of Capsicum annum L. were investigated by acetic acid-induced writhing, tail immersion and hot plate test. The anti-inflammatory activities were observed using carrageenan-induced edema of the hind paw in rats. Animals were divided into ten groups (n=7): (1) Control (2) CAG 200 (3) CAG 400 (4) CAR 200 (5) CAR 400 (6) CAO 200 (7) CAO 400 (8) CAY 200 (9) CAY 400 and (10) Standard. All the extracts were given orally. Acute toxicity was also determined by increasing the dose to 3000 mg/kg, which showed no evidence of mortality. All extracts of Capsicum significantly increased the hot plate pain threshold and remarkably reduced the carrageenan-induced rat paw oedema. Results obtained were compared with the corresponding control group, revealing that the fresh fruit extract of all four kinds of bell pepper (200 mg/kg and 400mg/kg) possess anti-inflammatory and pain-suppressing activities possibly mediated via PG synthesis inhibition

Starr–edwards ball caged mechanical heart valves’ reverberations – need for a phoenix of analysis with a critical mindset

The mechanical heart valve prosthesis with a caged ball has been around for 60 years. It has since experienced changes and adjustments. The Starr–Edwards (SE) valve was a pioneer and was taken out of clinical use in the late 2000s. SE is reportedly close to or has reached the age of 50, according to numerous sources from around the globe. The author’s observations and review of the literature about thrombogenicity, pannus formation, left ventricular outflow tract obstructions, and infective endocarditis in the SE valves, which were reportedly higher before its clinical ending, were only an overestimation and were not supported by scientific data. However, it was less discussed in the article, along with potential benefits. The main reasons for its demise were its obtrusive size and unattractive shape. There have been several successful implantations of these valves in Asian nations and reports of fewer primary failures and unexpected cardiac fatalities. Therefore, there is a need for extensive data gathering, documentation, and more recent studies on these valves to prepare for a prospective revival of use with newer research, mainly when long-term durability is considered.

Assessment of the prevalence of menstrual complications with knowledge, attitude and practice regarding menstruation of rural girls in jashore, bangladesh

Context: Menstruation is one of the most basic characteristic features of girls and inadequate knowledge of adolescent girls about menstruation can result in faulty menstrual hygiene management (MHM) and inappropriate dietary management both of these can causes diseases which is a major threat for adolescent health in developing countries like Bangladesh. Aim: To identify current knowledge, attitudes, and practices related to menstruation and menstrual hygiene management (MHM) among adolescent girls. Settings and Design: It is a descriptive cross-sectional study conducted in Jashore, Bangladesh. Methods and Materials: A pre-formed, pre-tested questionnaire was used. The questionnaire-based survey was conducted among randomly selected 250 adolescent girls between 10 and 19 years. Statistical analysis used: Data were analyzed statistically by Microsoft office excels and SPSS windows version 16 software programs. Results: Among 250 adolescent girls majority had their menarche between 12 and 15 years of age, 70% of adolescent girls of this study belonged to lower class families. This study revealed that 85% of the respondents had regular menstruation whereas 15% had irregular menstruations also. Most of them faced different types of Premenstrual syndrome including 76% from headache, 80% from lower abdominal pain, and 62.5% from fatigue and weakness. Only 56% had previous knowledge about puberty before attaining menarche. There was a statistically significant correlation between the mother’s education and the knowledge of participants about menstruation at puberty. All the participants used to bath and wash hand after changing pads whereas 80.8% used to avoid all types of exercises. Almost 94% of respondents used to dispose of their napkins properly. The micro-nutrient deficiency was also common; especially 49.2% were suffering from Iron deficiency and 69.6% from folic acids deficiency. Conclusion: Awareness programs regarding both menstrual hygiene and dietary management can aid to alleviate some complications during puberty and ensure better health for adolescent girls.

Review of mentoring in higher education

Mentoring relationships between a senior, experienced faculty member and a junior employee, between a faculty member and an undergraduate or graduate student, or between a senior and a junior student is essential in higher education. The current review aims to emphasize the necessity of establishing organized mentorship programs at national institutions to enhance academic performance, teaching/learning effectiveness, knowledge and skill development, growth and development, research productivity, and community engagement. The mentor and mentee both benefit from the mentoring relationship. Academic advising, preceptor supervision, and supervision of graduation projects are different from the mentoring relationship as they focus on only one aspect of the relationship, whether it is the study plan, the study session, or the project, respectively. Mentoring is a mutually beneficial relationship that calls for integrity, decency, self-assurance, trust, and respect. Educational institutes and colleges should set up a committee to establish a well-organized mentoring program with all the necessary templates for recruiting mentors and mentees, and also for annual evaluation and feedback from all parties involved.

Preparation and physicochemical and in-vitro evaluation of lomustine nanoparticles

In the present study nanoparticles of lomustine were fabricated using chitosan polymer crosslinked with different crosslinking agents like sodium tripolyphosphate and sodium hexametaphosphate. Different formulations of nanoparticles were prepared using different concentrations of crosslinking agents and polyethylene glycol 6000. The average particle size ranged between 112 nm to 942 nm. Zeta potential of nanoparticles ranged between 29.0 mV up to 56.0 mV. Encapsulation efficiency was variable from 58%-96%. The nanoparticles were solid spherical. In vitro drug release study was carried out in phosphate buffered saline solution pH 7.4 for 10 h. The analysis of regression values of Higuchi plot suggested diffusional mechanism and follows Fick's law of diffusion. Drug polymer interaction was absent as evidenced by FT-IR spectra and DSC thermograms. With polyethylene glycol inclusion shows interaction between lomustine and PEG. Cell viability assay (MTT Assay) showed that the lomustine nanoparticles were able to reduce the tumour cell proliferation and increased cell viability significantly (p< 0.05) as compared to pure drug in L 132 human lung cancer cell line

Removal of sulfur dioxide from air using a packed-bed dbd plasma reactor (pbr) and in-plasma catalysis (ipc) hybrid system

Sulfur dioxide, a noxious air pollutant, can cause health and environmental effects, and its emissions should be controlled. Nonthermal plasma is one of the most effective technologies in this area. This study evaluated the efficiency of a packed-bed plasma reactor (PBR) and in-plasma catalysis (IPC) in SO2 removal process which were finally optimized and modeled by the use of the central composite design (CCD) approach. In this study, SO2 was diluted in zero air, and the NiCeMgAl catalyst was selected as the catalyst part of the IPC. The effect of three main factors and their interaction were studied. ANOVA results revealed that the best models for SO2 removal efficiency and energy yielding were the reduced cubic models. According to the results, both PBR and IPC reactors were significantly energy efficient compared with the nonpacked plasma reactor and had high SO2 removal efficiency which was at least twice larger than that of the nonpacked one. Based on the results, the efficiency of IPC was better than in PBR, but its performance decreased over time. However, the PBR had relatively high SO2 removal efficiency and energy efficiency compared to the nonpacked reactor, and its performance remained constant over the studied time. In optimization, the maximum SO2 removal efficiency and energy efficiency were 80.69% and 1.04 gr/kWh, respectively (at 1250 ppm, 2.5 L/min, and 18 kV as the optimum condition) obtained by the IPC system which were 1.5 and 1.24 times greater than PBR, respectively. Finally, the model’s predictions showed good agreement with the experiments.

The modern filipina archetypes and ambiguities in the wildflower teleserye vis-à-vis filipinos’ sensibilities

The article is a discourse analysis of the Wildflower telerserye, exploring the themes and portrayals of the main character and the ambiguities projected by her characterization. The research’s objective revolves around the details that upon the attempt of the telersye to flaunt the modern archetypes of a Filipina, a strong and empowered woman who liberates under the yoke of patriarchal hegemony, her actions and ways were deemed crucial and subjected to great scrutiny. The researcher also dealt with how these portrayals affect and reflect the Filipinos’ way of thinking and sense of morality. Two major literary theories were used to underscore the article’s crucial points. To show how the woman portrayals in teleserye try to subvert the idea and configuration of patriarchy, Feminism (Helene Cixous) was used. Meanwhile, Deconstructionism (Jacques Derrida) was employed to show that in the attempt of the teleserye to empower the woman lead character, remove her from her marginalized and stereotypical views, and liberate her from the yoke of patriarchal hegemony, the result was otherwise. Based on these analyses, a synthesis of Filipinos’ sensibilities on how it affects and reflects their ways of thinking was crafted.
