Knowledge and practice of foot care among libyan diabetic patients

Diabetes mellitus is a common and chronic metabolic disorder. Uncontrolled diabetes mellitus leads to body organ damage if not treated properly. Educational intervention is necessary from health-care providers such as pharmacists to prevent or reduce organ damage and complications among the diabetic patients. Several studies have demonstrated the contribution of pharmacists in achieving a better control of diabetes mellitus. Thus, this study intended to assess the knowledge and practice regarding foot care among Libyan diabetic patients in Al-Bayda city. In addition, to determine the association between knowledge and practice regarding diabetic foot care with selected demographic variables. This is a cross sectional study conducted at Diabetes Center Aljabal Al akhdar in Al-Bayda between April and August, 2022. A total of 400 participants with diabetes were enrolled and interviewed by using a self-design questionnaire for knowledge and practice of foot care after taking verbal agreement. Data was analyzed by using SPSS software version 26.0. In this study, 191 subjects were male (47.8%) and 209 subjects (52.3%) were female. On the topic of foot care, 51.8% of the participants stated they did know how to perform correct foot hygiene and 33.5% stated that drying should consist of passing a towel between their toes. Female subjects statistically had more knowledge than male subjects on the right footwear and correct way of nail cutting (p < 0.01 and p < 0.001, respectively). In relation to practice, the frequency of patients with a good practice was 126 (31.5%) and with poor practice was 274 (68.5%). Evaluating the parameters dry, moisturize and nail cutting, women showed a significant difference in relation to men, with a high significance level in the three parameters. In conclusion, the knowledge and practice of foot care in Libyan patients with diabetes were poor. So, an educational program is recommended to improve the awareness of foot care. Improving quality work by infusing a “sense of belongingness” in lowest-level workers.

Objective: Improving quality work by infusing a “sense of belongingness” in lowest-level workers. The present work focuses on the management of human resources in an enterprise where contract workers are involved in carrying out quality work related to the parent organisation. In today’s world, where the number of supervisors has reduced significantly, it is a challenge to maintain the quality of work at a satisfactory level. The paper proposes to enhance the quality of work by infusing a “sense of belongingness” into lowest-level workers. Methodologies adopted: Case studies. The first case study was conducted between 2007-2011 during a project related to Rural Electrification Work in the Bokaro district under the scheme RGGVY. The author was deputed to supervise the rural electrification work of 300 villages with thousands of kilometres of 11kv and 415V distribution lines, along with over 400 distribution transformers in four blocks of the Bokaro district in Jharkhand. Contractors engaged local workers for erection of all infrastructure. The author explained the process of erection to local communities and brought about a sense of belongingness in them towards the infrastructure being developed. The second case review was conducted between 2013-16 at Chandrapura, Bokaro, during the operation and maintenance work of the 220KV switchyard. Six workers were involved in the maintenance work of the switchyard. They rectified faults during emergencies and took care of housekeeping. However, they only followed orders and never worked proactively. The author divided the workplace into eighteen parts, each maintaining three parts. During monthly walk-in inspections, one worker was awarded as the best contractor’s employee of the month of that section, which introduced a sense of competition among them. Analysis: During the first case study, after pointing out the benefits of the infrastructure being developed and how the quality of work will help in its sustainability for an extended period, villagers realised its importance in their well-being. As a result, they kept vigil over the contractor’s work during the erection process. In second case study, the repetitive external motivation (awards and appreciation) infused a “sense of belongingness” in them. Thus, all employees started functioning proactively. As a result, the occurrence of electrical faults was reduced drastically, and housekeeping improved. Findings: These two case studies lead the author to coin the term “sense of belongingness”, which can lead to improve the quality of work by the lowest-level workers in a company. There are five ways to develop a “Sense of Belongingness” (SOB) among workers- external motivation, which can lead to internal motivation; mutual respect; a sense of duty (every person has their own responsibility); brainstorming sessions (to make them feel as an integral part of the department); encouraging them to do more than expected. Conclusion: All these steps help to develop a “sense of belongingness” among the lowest-level workers in an organisation. Without these principles, it will be a challenge to achieve quality work. The paper addresses all the processes in detail to improve the work culture in a department and, ultimately, an organisation.

Qualitative assessment of the clinico-pathological features of highly pathogenic avian influenza h5n1 outbreaks in commercial poultry and peri-domestic birds in northern nigeria

The control of highly pathogenic avian influenza (HPAI) in Nigeria from inception is predicated on effective biosecurity by stamping out policy but outbreaks of the disease continued to re-occur with altered clinico-pathologic manifestations. This study undertook the qualitative assessment of the clinico- pathological features of HPAI H5N1 during the 2021/2022 outbreaks in commercial poultry and peri-domestic birds in northern Nigeria. A total of 22 commercial poultry farms with 53,932 laying chickens and 3 households with 120 backyard broiler chickens, 18 indigenous chickens, 10 peafowls and 9 geese were investigated for HPAI. The clinico-pathologic manifestations observed in commercial poultry were subtle compared to previous presentations of the disease except in the peafowls, geese, broilers and indigenous chickens. The interspecies mortality rates significantly varied from 1.6% to 19.6% for laying chickens and 33.3% to 100% for broilers, indigenous chickens, geese and peafowls. Based on the history of sudden and high mortality, clinical signs and post mortem lesions observed, three diseases; HPAI, very virulent Newcastle disease and fowl cholera were drawn out as differential diagnoses. However, a tentative diagnosis of HPAI was made and samples were sent to the National Veterinary Research Institute, Vom, Plateau State, Nigeria for confirmatory diagnosis. Results of the laboratory tests conducted on the samples using one step flu A screening and duplex real time RT-PCR and virus isolation in embryonated chicken eggs confirmed HPAI H5N1 in twenty-five farms. It is concluded that the continuous though, irregular outbreaks of HPAI and emerging clinico-pathologic manifestations are pointers to failure of control and that the disease may become endemic. It is recommended that government should review its policy on the control of HPAI to include the adoption of zoned vaccination with close monitoring.

Strategic management - concept of strategy

The strategy is a means of establishing the organisation's purpose in terms of priorities for resource allocation, action programs and long term objectives (Grant 2005). Chandler (1962) defines strategy as determining the primary long-term goals of an organisation, the adoption of courses of action, and the allocation of necessary resources. Besides, the strategy also focuses on allocating resources in relation to the organisation and its environment (Schendel & Hatten 1972). The strategy also pertains to the top management's decisions pertaining to its scope and the future direction. Such strategy covers an action plan on the organisation's future, commitments to specific products, services, and the markets.

The effect of cigarette smoking on the oxidant–antioxidant imbalance in patients with chronic obstructive pulmonary disease

Background Predisposing factors for chronic obstructive pulmonary disease (COPD) comprise endogenous genetic factors and exogenous factors such as positive and negative smoking, air pollution, inhalation of dust, and work-related chemicals. Because only a small percentage of smokers develop COPD, it is thought to arise as an outcome of environmental harm and a host’s response system, which boosts the tendency of the disease to arise. Objective To clarify the effect of smoking on the level of oxidant–antioxidant in patients with COPD. Patients and methods Thirty patients with COPD (20 smokers and 10 nonsmokers) and 40 healthy individuals (18 smokers and 22 nonsmokers) participated in this study. The investigations included measurement of malondialdehyde (MDA), superoxide dismutase (SOD) activity, catalase (CAT) activity, glutathione peroxidase (GPx), glutathione reductase (GR), vitamin C, and glutathione (GSH) content. Results SOD, CAT, GPx, GSH, GR, and vitamin C were significant lower (P<0.0001) among patients with COPD (smokers and nonsmokers) than in the control group, whereas the MDA levels were elevated (P<0.0001) in patients with COPD (smokers and nonsmokers) when compared with the control group. Moreover, the values of SOD, GSH, and GPx were significantly lower (P<0.001) in smokers with COPD as compared with nonsmokers with COPD. Finally, there was no significant difference in the level of MDA between patients with COPD (smokers and nonsmokers). Conclusion There is a significant reduction in the level of some antioxidants (SOD, GSH, and GPx) in smokers with COPD compared with nonsmokers with COPD, but other antioxidants (CAT, GR, and vitamin C) did not change significantly, whereas the oxidant levels (i.e., MDA) were relatively convergent in patients with COPD (smokers and nonsmokers). This, in turn, points to the modest role of smoking in the causation of oxidant–antioxidant imbalance in this diseas

Whatsapp- messenger fever on students

WhatsApp is mobile application which allows exchange of messages, videos, audio’s and images via Smartphone. The increased use of IM on phones has turned to be goldmine for mobile and computer age. This paper focuses on understanding experimental viewpoint about the intensity, usage of whatsapp messenger and its impact on the academic performance of students in institutions. Instead of fast communication and enhancing effective flow of information and idea sharing among students, whatsapp has actually impacted in some pessimistic performance of students. For instance it takes up much of the students study time resulting in procrastination related issues , destroys students’ linguistic skills, leads to lack of concentration during lectures, results in difficulty in balancing online activities (whatsapp) and intellectual preparation and distracts learner from completing their assignments and adhering to their private studies time table. It is very userfriendly and easy to get initiated. Simply enter the telephone number of the device into the app. It then sorts through the contacts (with your permission) on the phone to figure out who else also has the app already installed. Users can then invite more contacts or go ahead and start sending messages to the ones that the app discovered. Brian Acton and Jan Koum (2009) invented Whatsapp messenger for easy and fast communication and distribution of multimedia messaging. Whatsapp is one of the trend and fashion in technology that is commonly used on specific mobile phones and computers. Since the Smartphones became popular, many messaging services were launched but Whatsapp has become widespread among them. The service is available free for one year and later user has to pay very less annual amount. Besides all, this Application is highly fanatic and can create a great impact on regular users, and apart from that it can leave a trace that becomes difficult to control and cure. With whatsapp messenger, communication through mobile phones has become easier, faster and cheaper. It is less expensive as compared to the normal phone messaging. An individual can chat with friends and family overseas through whatsapp without having to incur global SMS charges.
