Association of vitamin d deficiency with dyslipidemia, glycemic control, and microalbuminuria in patients with type 2 diabetes mellitus in qatar

Background: Emerging data found that Type 2 diabetes mellitus (T2DM) is associated with Vitamin D deficiency at various frequencies. This study aims to estimate the prevalence of Vitamin D deficiency in T2DM patients in Qatar and the correlation between Vitamin D deficiency and other variables such as dyslipidemia, glycemic control, and microalbuminuria. Methods: This retrospective cross-sectional analytical study was conducted in the medical outpatient clinic at Hamad General Hospital. The study involved adult patients (≥18 years) with T2DM. The study covered patients admitted between January 1, 2018, and July 31, 2018. Ethical approval was obtained from the Medical Research Committee. Results: We recruited 400 subjects with T2DM. Their mean age was 58.97±10.32 years, and the majority were women (52.0%) and Arabs (69.5%). The mean duration of Type 2 diabetes diagnosis was 14.94±8.99 years. The prevalence of Vitamin D deficiency was 29.5%. A comparison between Vitamin D deficiency and non-vitamin D deficiency groups showed a statistically significant difference in terms of fasting blood (FB) sugar (p<0.001), random blood (RB) sugar (p<0.001), hemoglobin A1c (HBA1c) (p<0.001), total cholesterol (P = 0.001), low-density lipoprotein cholesterol (LDL [C]) (p=0.004), high-density lipoprotein cholesterol (HDL [C]) (p<0.001), triglyceride (p<0.001), and urinary albumin excretion rate (UAER) (p=0.007). Data analysis showed that a significant negative correlation was found between Vitamin D level and FB sugar (r=−0.208, p<0.001), RB sugar (r=−0.20, p<0.001), HBA1c (r=−0.260, p<0.001), total cholesterol (r=−0.218, p<0.001), LDL (C) (r=−0.176, p=0.004), triglyceride (r=−0.342, p<0.001), and UAER (r=−0.184, p=0.007). Conclusion: Our study showed a significant correlation between Vitamin D deficiency and the poor control of T2DM, dyslipidemia, and microalbuminuria. The results emphasize the importance of monitoring Vitamin D status in high-risk populations.

Bank liquidity risk and bank credit risk: implication on bank stability in ghana

Bank sector crisis across the globe is largely blamed on the joint effort of bank liquidity and bank credit risks. And so, the twin concepts of liquidity and credit risks have come under keen academic scrutiny, especially in investment finance. Contributing to the extant literature on these developments, secondary data were obtained from the websites of nine banks in Ghana, spanning 2008 to 2018, to determine how liquidity and credit risks separately and interactively impact bank stability in Ghana. Analysis of data was done using a panel regression through the fixed effects model after running the Hausman Test. The study confirms an inverse liquidity risk-bank stability relationship, emphasising the need to channel idle funds into interest-earning securities to consolidate bank profits. Although a further revelation suggests an insignificant negative relationship between credit risk and bank stability, it re-echoes the need to implement policy recommendations made by the Banks and Specialised Deposit-Taking Institutions’ ACT 2016 (ACT 930), section 62 of Ghana, on the threshold to lend funds to clients. The bank-size-stability relationship was positive. Increasing bank size through establishing more branches nationwide is encouraged but to a precautionary level since banks tend to suffer diseconomies of large scale operations due to unregulated expansion. There is the need to observe the Basel III provisions on maintenance of a 30-day optimum liquidity threshold of up to 100% and above. Besides, banks should tighten up their credit requirements and also ensure loan repayments history is monitored to benefit clients who are in good standing.

A study of wsn and analysis of packet drop during transmission

WSN is a low-power system and are often used in numerous monitoring uses, such as healthcare, environmental, and systemic health surveillance, in addition to military surveillance. It is important to reduce network resource usage since many of these applications need to be installed in locations that are virtually inaccessible to humans. Many protocols for WSN to extend the presence of the network have been established to solve this problem. In the energy efficiency of WSN networks, routing protocols play an important role since they help minimize power usage and response time and provide sensor networks with high data density and service quality. This study also employed a Hopfield neural network and the findings from this study are presented next to each other to enable comparison. This paper also discusses how to easily and accurately capture and handle WSN collisions. Future experiments that require the usage of neural networks and so many fuzzy structures will be able to prevent a crash in these respects.

Assessment of knowledge and attitude of pharmacists toward the side effects of anesthetics in patients with hypertension: a cross-sectional study

Pharmacists are expected to play major roles as members of diverse perioperative care teams besides implementing medication safety standards, pharmacist’s roles include optimizing drug therapy and other clinical interventions. Yet, developing general perioperative management as another crucial role operation room, pharmacists play and incorporating it into pharmaceutical education would be important. Enriching perioperative care provided by pharmacists can contribute toward improving the clinical competence of these professionals. Presently, there are no pharmacists in Zawia who participate in perioperative care. The objective of this study was to assess the pharmacist's knowledge of general anesthesia and drugs used to anesthetize patients who suffer from hypertension. A cross-sectional study was conducted in Zawia City, Tripoli in 2023. A self-designed questionnaire was distributed to 100 pharmacists. The study found that more than half of the pharmacists have humble knowledge about the drugs used in anesthesia, the findings indicated weakness in their information about drugs used to manage hypertension in surgery, and 36.0% know that sodium nitroprusside is used to control blood pressure during the operation. In conclusion, this study reported that some of the study's pharmacist participants had weak points that contributed to a high percentage of incorrect knowledge. These weak points may be attributable to poor educational preparation or a lack of training before the pharmacists began working in hospitals.

Normal cell differentiation potential of cancer stem cells without reprogramming pluripotent factors: a novel strategy in stem cell-based therapy for tissue regeneration

Stem cells hold great promise for tissue regeneration and have the potential to treat many incurable degenerative diseases. Cancer stem cells (CSCs), or cancer initiating cells, have the ability to self-renew and differentiate into heterogeneous lineages of cancer cells. Current stem cell therapies face limitations, such as limited stem cell sources, time consumption, tumor formation, and immune rejection upon allogeneic transplantation. Allogeneic stem cell treatments simplify stem cell manufacturing and reduce transplant time, but their therapeutic potential is limited by human leukocyte antigen (HLA)-matched donors. CSCs retain characteristics essential for tissue regeneration. However, several limitations hinder cancer stem cell reprogramming with pluripotent factors. The development of 3D culture models for tissue imitating extracellular matrix in cancer cell lines aims to enhance CSC enrichment. This mini-review focuses on a new strategy for treating incurable degenerative diseases involving in vitro and in vivo 3D cancer models and the induced differentiation of CSCs into mature normal cell types. This allows tissue survival without immune rejection and offers a safe alternative to cancer stem cell reprogramming with pluripotent factors. In conclusion, preservation and banking of allogeneic CSCs offer an alternative, readily available, and safe strategy that can be used to facilitate stem cell-based cell therapy.

Acceptance of covid-19 vaccine among general population and students in benghazi, libya: a cross sectional study

The most successful and cost effective method for control the COVID-19 pandemic is the vaccination. Currently, there are different COVID-19 vaccine introduced into Libyan market. This study aimed to evaluate and compare the acceptance of COVID-19 vaccine among general population and students in Benghazi city. A cross sectional survey was conducted during March 2021 among 440 respondents (240 general populations and 200 students) using a self-administered questionnaire. The results were considerably similar among the two groups of participants. Only 10.9% said that they infected with COVID-19 during the pandemic period. About two thirds of the respondents agreed to be vaccinated against COVID-19 when available whereas the majority said they would wait some time before taking the vaccine (71.4%). Approximately, one third of the participants preferred the Sputnik vaccine, 24% preferred the Pfizer and BioNTech and 19.2% preferred Oxford (AstraZeneca) vaccine. More than a half (55.9%) of the participants are refused to pay for the vaccine. The majority of the respondents do not think the vaccine alone will be enough in protection against the COVID-19 (79.8%). There was no significant association between general population, students and the COVID-19 vaccine acceptance (p = 0.776). Additionally, there was no significant correlations between socio-demographic data and attitudes of the respondents with the acceptance of COVID-19 vaccine (p > 0.05), except for using hand sanitizers daily was significantly associated among the general population (p = 0.008). In conclusion, although, the general population and students have a good acceptability to the COVID-19 vaccine, more education health programs and promotion are wanted to reduce the vaccine hesitancy by declaring the importance of the vaccine, showing sufficient data stabilize the vaccine safety.

Pengembangan karakter mandiri melalui pembelajaran tematik pada kelas iii di madrasah ibtidaiyah negeri 5 jember

Fungsi pendidikan adalah untuk mengembangkan karakter peserta didik. Oleh karenanya, guru dituntut agar dapat mengembangkan karakter melalui proses pembelajaran diantaranya melalui pembelajaran tematik. Salah satu sikap yang dapat dikembangkan melalui pembelajaran tematik adalah karakter mandiri. Penelitian ini bertujuan untuk mendeskripsikan perencanaan, pelaksanaan serta faktor-faktor yang mempengaruhi pengembangan karakter mandiri melalui pembelajaran tematik pada kelas III di Madrasah Ibtidaiyah Negeri 5 Jember. Penelitian ini menggunakan pendekatan kualitatif dengan jenis fenomenologis. Teknik pengumpulan data menggunakan observasi, wawancara, dan dokumentasi. Hasil penelitian ini adalah: 1) perencanaan pembelajaran tematik dalam pengembangan karakter mandiri di Madrasah Ibtidaiyah Negeri 5 Jember diwujudkan dalam RPP yang disusun secara sitematis dan berbasis karakter dengan memasukkan karakter mandiri pada komponen kompetensi inti, kompetensi dasar, indikator, kegiatan pembelajaran, dan penilaian; 2) pelaksanaan pengembangan karakter mandiri pada pembelajaran tematik dilakukan melalui kegiatan pembelajaran mulai dari kegiatan pendahuluan, inti dan penutup. Namun kegiatan inti lebih dimaksimalkan dengan menggunakan model learning by doing dengan melakukan lima aktivitas belajar yaitu menanya, mengamati, mengumpulkan informasi, mengasosiasi, dan mengkomunikasikan; dan 3) faktor-faktor yang mempengaruhi pengembangan karakter mandiri yaitu lingkungan keluarga, lingkungan sekolah dan lingkungan masyarakat.

Novel clinical pharmacy practice: extended role and improved competencies

In health care delivery systems, if you are not part of the solution, then you must be part of the problem. Over the last few decades, clinical pharmacy encourages pharmacists and pharmacy support staff to move their focus from product-oriented role towards new direct engagement with patients, to make the most of the benefits that patients obtain from the medicine they take, or the problems they encounter with their medicines use. In the USA, pharmacists’ participation in physicians ward rounds was shown to reduce adverse drug events by 78% and 66% in general medical and intensive care settings [1. 2]. A study covering 1 029 US hospitals indicated that centrally based and patient-specific clinical pharmacy services are associated with reduced mortality rates [3]. The services involved were medicine information, clinical research performed by pharmacists, active pharmacist participation in resuscitation teams and pharmacists undertaking admission medication histories. New pharmacists’ roles in healthcare systems around the globe have moved and developed significantly over the past few years, particularly with expansion of the scope of practice which allows pharmacists to focus on the clinical aspects of direct patient care [4, 5]. Pharmacists are drug therapy experts of health care team. Therefore, pharmacists, today, are the arbiters of effective and safe use of medicines. Medication therapy management is one of the major areas in which physicians more and more rely on pharmacists to benefit their patients.

Factors influencing acceptance of newborn screening for sickle cell disease in bauchi state, nigeria

Background: Sickle cell disease (SCD) is one of the most common genetic diseases worldwide, with very high prevalence rates in sub-Saharan Africa, especially Nigeria. Given the importanceand benefits of newborn screening (NBS), it is essential to assess awareness and acceptance of NBS for SCD and the factors influencing its acceptance in this region. Methods: This was a hospital-based, cross-sectional study that examined pregnant women attending the antenatal clinic for their first visit between August 1 and October 31, 2023. This antenatal clinic has an average annual attendance of 1,521 pregnant women who come for their first visit. The data obtained were analyzed using IBM SPSS Statistics (version 20), and the results were presented in prose, tables, and figures as appropriate. A level of statistical significance was set at p < 0.05. Results: Three hundred and two (302) pregnant women were recruited for the study. Though there was low awareness (13.2%), there was a high acceptance rate (96.7%) of NBS for SCD amongst the women studied. However, the type of marriage (monogamous marriage, p = 0.029) was the only factor found to have a significant relationship with the respondents’ willingness to accept NBS for SCD. Conclusions: Community education, with an emphasis on married couples, women of reproductive age, and religious leaders, should be intensified. The monogamous marriage setting may reflect marital stability, social support, or correlated socioeconomic factors on health decision-making. However, this may have different implications in different societies.

This new trend in creative cultural tourism has emerged from the changes in the production of cultural tourist commodities, the skilled tourist activities, and new consumption patterns. Moreover, creative cultural tourism can solve those problems experienced by traditional cultural tourists. A literature review confirmed that a move to creative cultural tourism, or intangible heritage (i.e., linguistic diversity or gastronomy), from tangible cultural attractions (i.e., museums, monuments, and so forth) increases the attractiveness of tourism destinations. In addition, this study explains the transformation from traditional cultural to creative cultural tourism.

N-gram-based machine learning approach for bot or human detection from text messages

Social bots are computer programs created for automating general human activities like the generation of messages. The rise of bots in social network platforms has led to malicious activities such as content pollution like spammers or malware dissemination of misinformation. Most of the researchers focused on detecting bot accounts in social media platforms to avoid the damages done to the opinions of users. In this work, n-gram based approach is proposed for a bot or human detection. The content-based features of character n-grams and word n-grams are used. The character and word n-grams are successfully proved in various authorship analysis tasks to improve accuracy. A huge number of n-grams is identified after applying different pre-processing techniques. The high dimensionality of features is reduced by using a feature selection technique of the Relevant Discrimination Criterion. The text is represented as vectors by using a reduced set of features. Different term weight measures are used in the experiment to compute the weight of n-grams features in the document vector representation. Two classification algorithms, Support Vector Machine, and Random Forest are used to train the model using document vectors. The proposed approach was applied to the dataset provided in PAN 2019 competition bot detection task. The Random Forest classifier obtained the best accuracy of 0.9456 for bot/human detection.

Utility-based joint power control and resource allocation algorithm for heterogeneous cloud radio access network (h-cran)

The high density of H-CRAN associated with frequent UE handover may degrade the throughput. The infrastructure equipment like RRHs and BBUs consumes more energy to reduce UE energy consumptions. In this paper, we propose a utility-based joint power control and resource allocation (UJPCRA) algorithm for heterogeneous cloud radio access network (H-CRAN). In this framework, the power consumption of baseband units (BBUs), remote radio heads (RRHs), and macrocell base station (MBS) are estimated by predicting their dynamic loads. The data rate achievable for UE associated with each RRH and MBS on resource block RBk is then estimated. The user wishing to connect to a RRH or MBS then checks the corresponding utility with minimum expected energy consumption and the maximum expected data rate. If any UE with high priority traffic connected to MBS could not achieve its desired data rate requirements, then it can cooperatively seek the assistance of any RRH for assigning the balance RBs. The throughput may be enhanced by the high density of H-CRAN and frequent UE handover. Inter- and intracell interference causes the H-CRAN macrocells’ improved data rate to diminish. To lower UE energy consumption, infrastructure devices like RRHs and BBUs need more energy. As a result, there is a trade-off between operators and UE energy conservation. It is possible to determine the power consumption of BBUs, RRHs, and MBS using predictions of their dynamic loads. The UE may then forecast the data rate for each RRH and MBS on the resource block. When a user wishes to connect to an RRH or MBS, they look at the utility with the highest expected data rate and the least predicted energy usage first. A UE with high priority traffic connected to the MBS can cooperatively ask any RRH for assistance in allocating the remaining RBs if it is unable to achieve its intended data rate needs. Experimental results have shown that the proposed JRAUA algorithm achieves higher throughput, resource utilization, and energy efficiency with reduced packet loss ratio, when compared to the existing techniques.

The research method with qualitative, data collection with interview, observation, and documentation. The results showed (1) character planting through intracurricular activities with (a) strengthening learning, namely character building in accordance with the theme or material, and (b) spiritual paradigm by inserting Islamic values ??in the learning process. (2) character planting through extracurricular activities with (a) scouts, namely forming leadership characters, being independent, solving problems, and loving nature, (b) leadership, forming leadership characters, responsibility, and making decisions, (c) painting, forming detailed, focused, creative characters , (d) martial art, forming tough, skilled characters, (e) reciting recitation, forming religious, moral, orderly characters, (f) child preachers, forming brave characters, positive thinking, art of communication, influencing others. Pendidikan karakter merupakan upaya membina dan membentuk karakter siswa sesuai dengan budaya sekolah. A study to assess the effectiveness of infrared radiation therapy on pain perception and wound healing among primi postnatal women with episiotomy

Motherhood has true nobility and unique capacities. Pregnancy and childbirth are special events in women’s lives. This study aimed to assess the effectiveness of Infra-red radiation therapy on pain perception and wound healing among primi postnatal women with episiotomy in Christian mission hospital, Madurai. The study was conducted among 60 postnatal women, 30 in experimental group and 30 in control group, who were selected by using purposive sampling technique. Data collection was done as planned 6 weeks were taken for data collection procedure. The data gathered were analyzed and the interpretation was made on the study objectives. The paired‘t’ test and independent‘t’ test were used to find out the effectiveness of infrared radiation therapy. Comparison of pain perception and wound healing status values between pre-test and post-test, experimental and control group showed a significant difference at 0.05 levels. The study concluded that the infrared radiation therapy was effective in reducing episiotomy pain and wound healing. However, the Shiroro hydro plant, one of the main facilities located on the Kaduna River, is currently facing operational obstacles due to deteriorating infrastructure and inadequate maintenance practices. To overcome these challenges and improve efficiency within Nigeria's hydroelectric power sector, a hybrid-optimization approach has been proposed. This study sought to enhance the efficiency of the Shiroro hydro plant by implementing this innovative method. To achieve our objectives and address pertinent research questions, a mixed research method combining primary and secondary data was employed. The analysis included hydropower modeling and hydro-turbine input-output modeling. Three optimizer models, namely the particle swarm optimizer (PSO), Ant colony optimizer (ACO), and Artificial bee colony optimizer (ABCO), were utilized to formulate objective functions and task representations. The study involved comparing the daily output and fitness response of the Shiroro hydro plant through swarm optimizer iterations. The findings revealed a clear correlation between the turbine's power output and the water flow rate and water column height, suggesting that altering these factors could significantly improve the plant's performance. The comparison of the PSO, ACO, and ABCO models demonstrated that PSO and ABCO generated optimal or near-optimal solutions, while ACO produced suboptimal results. Consequently, the study concluded that enhancing the Shiroro hydro plant's output was feasible by increasing the water flow rate and column height. Additionally, the utilization of PSO and ABCO models proved to be an effective means of accurately predicting the turbine's output. As a result, the study recommended the integration of hybrid optimization techniques to monitor and identify any deviations in the Shiroro hydro plant's daily power output. This approach would enable prompt maintenance to be carried out, preventing significant damage to the plant. Ultimately, this research contributes valuable insights into improving the efficiency and performance of Nigeria's Shiroro hydro plant.
