Systemic onset juvenile idiopathic arthritis (SoJIA) is a rare inflammatory disorder. It is the severest form of juvenile idiopathic arthritis, and complications occur most commonly in this type. Non-responsiveness to standard therapy with corticosteroids and disease-modifying antirheumatic drugs is not uncommon. Interleukin-1 beta (Il-1β) has been shown to be a main contributor to the pathogenesis of SoJIA. Anakinra, a recombinant Il-1β receptor antagonist, was shown to be effective in small cohorts of therapy-resistant adult and pediatric still's patients. This study aimed to evaluate the real-world efficacy, steroid-sparing effect, and safety profile of anakinra in patients with SoJIA at a tertiary care center in Libya. A retrospective case series was conducted on patients with SoJIA treated with anakinra at the Tripoli Children's Hospital between 2010 and 2017. Data on demographic characteristics, disease activity, corticosteroid dosage, concomitant medications, and adverse events were collected at baseline and at 1-, 3-, 6-, and 12-month post-treatment. 13 patients were treated with anakinra with a mean age of 9.4±4.6 years at anakinra initiation and a female-to-male ratio of 2: 1. All patients were on corticosteroids and 92.3% on methotrexate at treatment initiation. The proportion of patients achieving inactive disease (Jadas 10=0) increased over time. A marked steroid-sparing effect was observed: the number of patients requiring high-dose steroids (>0.5 mg/kg/day) decreased from 100% at baseline to 7.6% at 12 months, and 53.8% successfully discontinued corticosteroids entirely. All patients experienced injection site reaction, and macrophage activation syndrome occurred in 15.4% as a side effect after treatment initiation. But no severe infections or fatalities occurred. Reasons for discontinuation included remission (46.1%), drug unavailability (23.0%), inefficacy (15.3%), and side effects (15.3%). Anakinra demonstrated significant efficacy in inducing rapid disease control and reducing corticosteroid dependence in patients with refractory SoJIA, with a manageable safety profile

The 2019 coronavirus (COVID-19) pandemic is spreading worldwide, with a spectacular increase in death missing any effective therapeutic treatment up to now. Molecular docking is a recognized computational tool to assist in early drug discovery and development. Molecular docking analysis was carried out using 2-(4-(aminomethyl)phenyl)isoindoline-1,3-dione Schiff base conjugates with SARS-CoV-2 protease enzyme and COVID-19 main protease in apo form 6M03). The compounds with the best normalized docking scores to protease enzyme (6LU7) were ARG3 (-8.1 kcal/mole), ARG7 (-8.1 kcal/mole) and ARG6 (-8.0 kcal/mole). The best docking ligands for main protease in apo form (6M03) were ARG7 (-8.7 kcal/mole), ARG6 (-8.6 kcal/mole) and ARG3 (-8.4 kcal/mole). The structural similarity between these conjugates inspired us to perform in silico studies to check their possible binding interactions with essential SARS-CoV-2 proteins. These studies provide insight into the potential binding between Schiff base derivatives and SARS-CoV-2 proteins to provide an insight for finding an effective therapy. Finally, ADMET calculations were performed for the Schiff base compounds to predict their pharmacokinetic profiles.

Abstract: Recent studies on hydrogels have shown them as promising biomaterials for numerous applications involving tissue engineering, drug-screening, drug-delivery, and 3-D bioprinting because they show unique physicochemical properties. The ability of these structures to hold large amounts of water is because of their hydrophilic nature that provides a soft and hydrated environment like natural tissues. This makes them ideal for mimicking the extracellular matrix and supporting cell growth and proliferation. In tissue engineering, hydrogels might be used to create scaffolds that promote cell growth and facilitate tissue regeneration. Hydrogels can also be engineered in such a way that they intimate the mechanical and biochemical in vivo characteristics making them a versatile tool for applications in tissue engineering. Hydrogels are being used in drug screening, as they can be functionalized with different biochemicals in order to match the microenvironment of specific tissues. This allows researchers to study how drugs interact with cells and tissues in-vitro conditions, which can lead to more efficient strategies for drug development. For applications in drug delivery hydrogels are designed to release drugs in a sustainable and controlled way, improving the drug efficacy and reducing the toxicity of drugs. Designing can also be done in a way that they can target specific tissues and cells making them a promising tool for personalized medicine. Hydrogels are being used in 3-D bioprinting, where they serve as bio-inks that can be fabricated into complex structures with high precision. In comparison to conventional technologies, this is a promising technique that allows the construction of complex three-dimensional structures in a sequential manner by a computeraided system. One major challenge in bioprinting is finding such material that is suitable for printing and also satisfies the mechanical strength requisite for tissue engineering applications. That is where hydrogels serve as the most appropriate model and have encouraging or favorable operation potential as cell-affable materials. This technique has revolutionized tissue engineering by allowing researchers to create functional tissues and organoids and spheroids. Overall, hydrogel-based tissue engineering, drug screening, drug delivery, and 3D bioprinting are exciting areas of research with great potential to significantly impact different areas of medicine and biology.
In this article, we form the exact wave solutions of the Jimbo-Miwa equation and the Calogero-Bogoyavlenskii-Schiff equation by applying the new generalized (G'/G)-expansion method. We explained the new generalized (G'/G)-expansion method to look for more general traveling wave solutions of the above mentioned equations. The traveling wave solutions attained by this method are in terms of hyperbolic, trigonometric and rational functions. The graphical representation of the obtained solutions is kink soliton, singular kink soliton, singular soliton and singular periodic solution. This method is very significant for extracting exact solutions of NLEEs which habitually occur in mathematical physics, engineering sciences and applied mathematics.

Accurate diagnosis of the cause of acute or chronic kidney dysfunction may require a percutaneous kidney biopsy (PKB). Unfortunately, the invasive nature of the procedure can lead to potential complications that may discourage the KBs. Lack of appropriate communication skills, experienced personnel and equipment, and high procedure costs can negatively impact complication rates and the frequency of conducted KBs. This nonsystematic review assesses KB procedures, indications, contraindications, complications, post-KB monitoring time, and cost. We looked for reviews and original articles published between January 2010 and Jan 2025 on Google, Google Scholar, and PubMed. Different keywords, phrases, and sentences include PKB, renal biopsy, native PKB, ultrasound guide, CT-guided, PKB, allograft PKB, and PKB procedures. KB makes histopathological and immunohistological diagnosis possible, which are necessary for diagnosis and treatment. It is often used despite the known complications. KB and other biochemical assays have tracked transplant rejection and antirejection drugs. Automatic gun spiral needles sizes 14 and 16 capture enough samples with fewer complications than gauge sizes 18 and 20. KB cost is another issue, especially in low-income areas, and deserves additional study.
Melilotus officinalis (L.) Pallas. is used traditionally to treat hypertension and chronic venous insufficiency. The high prevalence of hypertension and the side effects of antihypertensive drugs potentiate us to evaluate Melilotus officinalis. The hypotensive effect of Melilotus officinalis butanol fraction (MOBF) was investigated in anaesthetised normotensive rats. The doses of 60, 80 and 100 mg/ kg, i.v., produced a significant (P<0.05) decrease in blood pressure. The statistically non-significant reduction in blood pressure was shown by 20 and 40 mg/ kg, i.v. These results suggest that Melilotus officinalis exhibited a hypotensive effect in normotensive rats. However, further evaluation of compound-related activity is required.
The domestication of animals and cultivation of crops have been essential to human development throughout history, with the agricultural sector playing a pivotal role. Insufficient nutrition often leads to plant diseases, such as those affecting rice crops, resulting in yield losses of 20-40% of total production. These losses carry significant global economic consequences. Timely disease diagnosis is critical for implementing effective treatments and mitigating financial losses. However, despite technological advancements, rice disease diagnosis primarily depends on manual methods. In this study, we present a novel Self-Attention Network (SANET) based on the ResNet50 architecture, incorporating a kernel attention mechanism for accurate AI-assisted rice disease classification. We employ attention modules to extract contextual dependencies within images, focusing on essential features for disease identification. Using a publicly available rice disease dataset comprising four classes (three disease types and healthy leaves), we conducted cross-validated classification experiments to evaluate our proposed model. The results reveal that the attention-based mechanism effectively guides the Convolutional Neural Network (CNN) in learning valuable features, resulting in accurate image classification and reduced performance variation compared to state-of-the-art methods. Our SANET model achieved a test set accuracy of 98.71%, surpassing current leading models. These findings highlight the potential for widespread AI adoption in agricultural disease diagnosis and management, ultimately enhancing efficiency and effectiveness within the sector.
