Bleomycin pulmonary toxicity in patients with germ cell tumors treated with bleomycin containing regimens: experience with 4 cases

Background: Information on Bleomycin pulmonary toxicity (BPT) in Qatar is scarce. The aim of this study was to estimate the prevalence of BPT and to describe its clinical significance and outcome in germ cell tumor (GCT) patients who received bleomycin-containing regimens.Methods: This retrospective cross-sectional study was conducted at the National Center for Cancer Care and Research. It included all patients diagnosed with GCT and treated with a bleomycin-containing regimen between January 2002 and December 2008 Results: We identified fourteen patients with GCT who received bleomycin containing regimen. Four of them (28.5%) had developed BPT, and they were males with mean age of 39.3±8.3 years (range: 25-46 years). The calculated creatinine clearance before treatment was normal in the 4 cases. Evaluation of the chest computed tomography scan before starting bleomycin containing regimens revealed that none of our patients had pre-existing parenchymal lung disease. The mean cumulative bleomycin dose was 187.5± 153.7 U, while the mean time to onset of BPT was 3.5±2.1 months. Once the diagnosis of BPT was established, bleomycin was discontinued in the four patients and short courses of dexamethasone were administered. Two patients (50%) died, while one patient survived with a fibrosis sequel, and the fourth patient recovered without a fibrosis sequel. Conclusion: BPT is one of the life-threatening side effects of this drug that every doctor should be aware of when treating GCT, therefore, a high index of suspicious is needed for early recognition of BPTs.

Characteristics-centered cryptography with cloud revocation authority and its applications

Identity-based encryption (IBE) is a public key cryptosystem and eliminates the demands of public key infrastructure (PKI) and certificate administration in conventional public key settings. Because of the absence of PKI, the revocation drawback could be a crucial issue in IBE settings. Many rescindable IBE schemes are planned relating to this issue. Quite recently, by embedding associate outsourcing computation technique into IBE, Li et al. planned a rescindable IBE theme with a key-update cloud service supplier (KU-CSP). However, their theme has 2 shortcomings. One is that the computation and communication prices are above previous rescindable IBE schemes. The opposite disadvantage is lack of measurability within the sense that the KU-CSP should keep a secret worth for every user. Within the article, we have a tendency to propose a replacement rescindable IBE theme with a cloud revocation authority (CRA) to resolve the 2 shortcomings, namely, the performance is significantly improved and the CRA holds only a system secret for all the users. For security analysis, we demonstrate that the proposed scheme is semantically secure under the Decisional Bilinear Diffie-Hellman (DBDH) assumption. Finally, we have a tendency to extend the planned rescindable IBE theme to gift a CRA-aided authentication theme with period-limited privileges for managing an outsized range of varied cloud services.

On the impact of debris accumulation on power production of marine hydrokinetic turbines: insights gained via les

We present a series of large-eddy simulations to systematically investigate the impact of debris accumulation on the hydrodynamics and power production of a utility-scale marine hydrokinetic (MHK) turbine under various debris loads lodged on the upstream face of the turbine tower. The turbine blades are modeled using turbine resolving, actuator line, and actuator surface methods. Moreover, the influence of debris on the flow field is captured by directly resolving individual logs and employing a novel debris model. Analyzing the hydrodynamics effects of various debris accumulations, we show that an increase in the density of debris accumulation leads to more flow bypassing beneath the turbine blade. This, in turn, reduces the flow momentum that reaches the MHK blades at the lower depths, inducing significant fluctuation in power production. Further, it is shown that debris-induced turbulent fluctuations contribute to significant variability in the MHK turbine’s power production.

Inhibitory potential of phaseolus lunatus l. seeds to digestive enzymes

Introduction: Study was conducted with an objective of investigating the inhibitory effect of Phaseolus lunatus seeds extract on three enzymes (α-amylase, maltase and glucoamylase) involved in diabetes. The disadvantages related to the use of synthetic drugs make it necessary to search for alternative medicines from medicinal plants. One therapeutic approach for treating postprandial hyperglycaemia is to decrease the plasma glucose level, which can be attained by lowering the starch hydrolysis by digestive enzymes such as α-amylase, maltase and glucoamylase. The use of synthetic antidiabetic agents caused gastrointestinal side effects. Methods: Hence, the research was aimed to evaluate the potential of Phaseolus lunatus seeds extract to inhibit α-amylase, maltase and glucoamylase as a possible mechanism of hypoglycemia caused by this plant with fewer side effects. Phaseolus lunatus, also known as Lima bean, is native to Tropical America and grown worldwide. Lima bean has been prescribed traditionally to treat different diseases, including diabetes. Result: The results of the present study showed that Phaseolus lunatus seeds extract has exerted both time and concentration-dependent Significant inhibitory effects on these digestive enzymes might be due to the cumulative impact of phytoconstituents in the plant extract. Conclusion: Therefore, it can be said that the part mechanisms by which Phaseolus lunatus exhibited its antidiabetic activity might be through inhibiting this digestive enzyme.

This research was conducted at SD Darus Sholah, Jember which aims to answer the main problems related to the implementation of the Student Team Achievement Divisions learning strategy in integrated thematic learning. The problem studied in this research is the Implementation of the Student Team Achievement Divisions Strategy in integrated thematic learning at SD Darus Sholah, Jember. This research approach uses a qualitative approach with the type of phenomenological research. Methods of data collection using interviews, observation and documentation. Data analysis used the interactive model of Miles and Huberman, with the process of data collection, condensation, data display, and data verification. To test the validity of the data using triangulation. The results of this study indicate that: 1) At the presentation stage, the teacher starts with story material, reviews previous material, and learning activities in teams. 2) The teacher prepares a worksheet as a guide for group work and each member can contribute, the teacher makes observations, provides guidance, motivation, and assistance if needed. 3) Individual tests in groups. 4) Score development for individuals, the teacher combines the previous score with the final score. 5) Appreciation for the team is based on the assessment of individuals in the group so that the process of group assessment recapitulation is based on individual assessment of each group. Pendekatan penelitian ini menggunakan pendekatan kualitatif dengan jenis penelitian fenomenologis. Metode pengumpulan data menggunakan wawancara, observasi dan dokumentasi. Analisis data menggunakan model analisis interaktif Miles dan Huberman, dengan proses data collection, condensation, data display, and data verifiying. Untuk menguji keabsahan data menggunakan trianggulasi. Hasil penelitian ini menunjukkan bahwa: 1) Pada tahap presentasi, guru memulai dengan materi cerita, mereview ulang materi sebelumnya, dan kegiatan belajar dalam tim. 2) Guru menyiapkan lembaran kerja sebagai pedoman kerja kelompok dan setiap anggota dapat berkontribusi, guru melakukan pengamatan, memberikan bimbingan, motivasi, dan bantuan jika diperlukan. 3) Tes individu dalam kelompok. 4) Skor pengembangan bagi individu, guru menggabungkan skor nilai sebelumnya dengan skor akhir. 5) Penghargaan bagi tim mendasarkan kepada penilaian terhadap individu dalam kelompok sehingga proses rekapitulasi penilaian kelompok, didasarkan kepada penilain individu masing-masing kelompok.

Association between oral health literacy, gingival health and oral hygiene among dental patients

Objective: To determine the association between oral health literacy, oral hygiene and gingival health status. Methods: This cross-sectional study was conducted among patients attending University of Benin Teaching Hospital, Benin City. Data were collected through interviewer-administered questionnaires. Index used in this study for estimating oral health literacy levels was Rapid Estimate of Adult Literacy in Dentistry- 30 (REALD-30). The participant’s oral hygiene status was assessed using the Simplified Oral Hygiene Index while gingival health was assessed using the Gingival Index. Results: Two hundred and eight participants with 130 (62.5%) males and 78 (37.5%) females formed the population of the study and their mean age was 28.32 ± 10.5 years. The prevalence of low oral health literacy was 86 (41.3%). The oral hygiene status of most of the participants 100 (48.1) was fair. The prevalence of gingivitis was 58.2%. Oral health literacy had significant statistical negative correlation with age, oral hygiene and gingival health status. Oral health literacy consistently emerged as a predictor of oral hygiene and gingival health status. Conclusion: Majority of the participants with low oral health literacy had fair/poor oral hygiene status (p = 0.000, OR= 17.870, 95% CI = 7.320-43.627) and gingivitis (p = 0.000, OR= 7.054, 95% CI = 3.514-14.164).

This paper aims to explore the supporting and inhibiting factors in the effort to integrate character education in akidah akhlak subjects at MIN 11 Aceh Tenggara. This study uses a qualitative method with a narrative approach and data collection techniques of interview, observation, and documentation; then, the data is analyzed using the methods developed by Miles and Huberman, namely data reduction, data presentation, and data verification. This study shows that the supporting factor for integrating character education in aqidah moral learning at MIN 11 Aceh Tenggara is the availability of learning media in the form of focus and Madrasah residents who work together to supervise all students, both outside and inside the Madrasah environment. The inhibiting factors are the limitations of Madrasah infrastructure in the form of no mosque or prayer room, family support, and social media and games that have an influence on student character.

In covid-19 pandemic; social support requirements and midwifery approaches to women during pregnancy, childbirth and the postpartum period

The novel coronavirus pandemic is affecting the whole world and it is predicted that the process will continue for a long time. The health, economic and social problems caused by this process have greatly affected women as well. In addition to the problems endured by the general population, women are facing additional challenges involving their reproductive health, pregnancy, childbirth and postpartum period. Women are trying to deal with a range of concerns, from difficulties in accessing family planning services to being unable to meet their own basic reproductive health needs. Under the pandemic conditions, it has become increasingly difficult for social services to respond sensitively to women's particular needs. The needs of coronavirus patients take precedence, and the women’s needs remain in the background. Women are facing stress, anxiety, fear, as well as disease. So, midwifery practices need to be revised to adapt to the pandemic conditions. This revision must involve not only technical issues but also psychosocial adjustments. Women have been demanding home births on social media, and this is a subject that require some legal regulations. Respect for women's birth is one of their most fundamental rights. National criteria should be developed to accommodate the support structures needed for a home birth. The social support that midwives offer women during pregnancy, childbirth and the postpartum period, play a key role. Since it is important to increase the awareness of midwives in this regard, this article deals with women's needs for social support and possible midwifery approaches for supporting women through their pregnancy, childbirth, and the postpartum period.

Formulation design and evaluation of mucoadhesive buccal patch of ketorolac for the treatment of periodontitis

Ketorolac tromethamine, the non-steroidal anti-inflammatory drug, was formulated into mucoadhesive films to overcome the limitations in the currently available dosage and routes of administration which in sequence will increase patients’ compliance. Films were cast from organic and aqueous solvents using various bioadhesive polymers namely: HPMC E-15, Eudragit RLPO, Eudragit RSPO, Carbopol 934P. The prepared films were subjected to investigation for their physical and mechanical properties, swelling behaviors, in vitro bioadhesion, drug permeation via bovine buccal mucosa, and in vitro drug release. These properties were found to vary significantly depending on the preparation methods, the type of the polymers, and the ratio of addition of both plasticizers (i.e. polyethylene glycol and film forming agent. Formulation F4 is selected for best formulation because it shows the 98.85% drug release at time 6 hr. In addition, this film was subjected to in vitro and in vivo release. The obtained results indicate that the concentration of ketorolac in the oral cavity was maintained above 4.0 µg/ml for a period of at least 6 h. This film shows promising results for using the ketorolac mucoadhesive route of administration topically and systemically, and thus it will be subjected to clinical evaluation in future work.

The effect of formaldehyde on the liver of adult male albino rats and possible protective role of vitamin c

Background: Formaldehyde is found in different kinds of medicine and industrial products, cigarette smoke, and even numerous vegetables, fruits and seafood that have been illegally preserved with formalin. The water soluble vitamin C is a strong antioxidant that scavenges free radicals and other reactive nitrogen and oxygen species. Even in small amounts, vitamin C can defend nucleic acids, lipids and proteins against oxidative damage. Objectives: the aim of this study is to elucidate the possible changes that take place in the liver of adult male albino rats after intraperitoneal injection of formaldehyde and the possibility of oral vitamin C’s hepatoprotectivity against it. Design: forty-five adult male albino rats were utilized in this work. These animals were allocated randomly into three main groups. Group I (control groups) included 3 subgroups each contained 9 rats : –ve control received no treatment , +ve control which were injected intraperitoneally by 1 ml of distilled water for 10 days and vit.C +ve control group which received a daily dose of ascorbic acid (100mg/kg bw) dissolved in distilled water by gavage for 10 days. Group ΙΙ: contained 9 rats, which were injected intraperitoneally with a daily dose of formaldehyde (10mg/kg BW) for 10 days. Group IΙΙ: contained 9 rats that were injected intraperitoneally with a daily dose of formaldehyde (10mg/kg BW) concomitantly with daily dose of ascorbic acid (100mg/kg BW) by gavage for 10 days. By the end of the experiment, blood samples were collected for biochemical study of ALT (Alanine aminotransferase), AST (Aspartate aminotransferase) and albumin and all animals were anaesthetized by ether inhalation. Liver specimens were dissected out and weighed then subjected to histopathological, immunohistochemistry, and morphometrical examination. Results: Administration of formaldehyde at a dose of 10 mg/kg caused increase in serum activities of ALT and AST ,but regarding albumin ,it remained unaffected .It caused increase in the liver weights and induced several histopathological changes in the liver of adult male albino rats as congested dilated central veins, portal veins and blood sinusoids with increase in the thickness of the wall of the portal vein. Meanwhile, some hepatic lobules showed multiple necrotic foci around central and portal veins. On the other hand, vit.C partially improved the state of oxidative stress as evidenced by iNOS (inducible nitric oxide synthase) immunohistochemistry. It also reduced the degree of hepatic fibrosis as evidenced by Mallory trichrome histochemical staining. Conclusion: Exposure to formaldehyde led to pronounced hepatic damage which is partially limited by vit.C coadministration. Recommendations: Many special precautions should be taken to limit the occupational and environmental exposure and the level of food and water contamination with formaldehyde and use vit.C as a supplement to limit the toxic effects on the liver.

Solar Energy is the prime important source of energy, and it has continued to gain popularity globally. As of 2018, about 486 GW of solar PV was installed worldwide. One of the key requirements for socio-economic improvement in any nation of the world is the provision of dependable electricity supply systems. Recently, there is a massive growth in access to solar electricity in several Africa countries, notably South Africa, Egypt, Morocco, and Algeria. As a result, it decreases the global population without access to electricity with an appreciable value. This paper review the present state of solar energy capacity in the world also identifies vital approaches of improving their functionality, reliability, and affordability as well as the essential method that policymakers may implement in the future. The result shows that there is a recurrent growth in solar energy annually and as at the end of 2018 Asia with the largest capacity of 56.58%, while Central America and the Caribbean have the lowest installed capacity of 0.36%. As the request for an uninterrupted supply increases in different regions, progressive builders are embracing solar photovoltaics (PV) renewable energy as an option for their customers.

Learning is a conscious effort made by individuals to acquire various kinds of abilities (competencies), skills (skills), and attitudes (attitudes) through a series of learning processes that will ultimately result in behavioral changes in the individual. The learning process in schools tends to emphasize the achievement of changes in cognitive (intellectual) aspects which are carried out through various forms of approaches, strategies, and certain learning models. Learning systems that specifically develop affective abilities have received less attention, affective abilities are only used as a nurturing effect. This study aims to (1) find out whether online learning is able to bring out the affective abilities of prospective study program teachers (2) analyze the affective abilities of prospective teachers in online learning. The data used in this study is qualitative data. The technique used is a questionnaire. This questionnaire was given to respondents, namely students of PGSD teacher candidates at Muhammadiyah University of Pringsewu. The results of the study indicate that affective abilities can appear in online learning as evidenced by the results of the questionnaire that has been given to respondents. The indicators of affective ability in this study include attitudes, interests, self-concept, values, and morals, all of which are in the good range. This means that the results of students' affective abilities can be said to be valid or appear in online learning

Hematological and histological effect of fractionated neem leaf extract in healthy wistar rats

Introduction: In recent years, the growing research towards new drugs has been targeted on plant-based drugs, and Neem (Azadirachta indica) is one of the plants that have been extensively researched for its diverse medicinal properties. The study aimed to determine the effects of neem on the hematological parameters (total white blood cells, neutrophil, monocyte and eosinophil counts) and histology of some organs of rats. Methods: Fifteen healthy male Wister rats divided into control (Nm0) and experimental groups (Nm11 and Nm22). Control group 1 (Nm0) was given 100mg/200g body weight of normal saline orally twice daily; experimental group 2 (Nm11), 100mg/200g body weight neem extract twice daily for 11 days and experimental group 3 (Nm22), 100mg/200g neem leaf extract twice daily for 22 days. Total number of white blood cells (WBC), lymphocytes, neutrophils, monocytes and eosinophils, packed cell volumes (PCV) and histological changes in the spleen, liver and kidneys were evaluated. Results: There were no significant differences in mean values of the hematological parameters (total WBC; PCV; neutrophils, lymphocytes, monocytes and eosinophils). We observed the central vacuolation and accumulation of lymphocytes in the spleen, hypertrophy of the central vein in the liver and shrinking of the glomeruli and accumulation of the lymphocytes in the kidney using hematoxylin and eosin staining following prolonged administration of neem extract (Nm22). Conclusion: Prolonged administration of neem affected the histology of some organs of the rats more than the hematological parameters

Attention is all you need

The dominant sequence transduction models are based on complex recurrent or convolutional neural networks that include an encoder and a decoder. The best-performing models also connect the encoder and decoder through an attention mechanism. We propose a new simple network architecture, the Transformer, based solely on attention mechanisms, dispensing with recurrence and convolutions entirely. Experiments on two machine translation tasks show these models to be superior in quality while being more parallelizable and requiring significantly less time to train. Our model achieves 28.4 BLEU on the WMT 2014 Englishto-German translation task, improving over the existing best results, including ensembles, by over 2 BLEU. On the WMT 2014 English-to-French translation task, our model establishes a new single-model state-of-the-art BLEU score of 41.0 after training for 3.5 days on eight GPUs, a small fraction of the training costs of the best models from the literature.

An efficient deep learning approach for colon cancer detection

Colon cancer is the second most common cause of cancer death in women and the third most common cause of cancer death in men. Therefore, early detection of this cancer can lead to lower infection and death rates. In this research, we propose a new lightweight deep learning approach based on a Convolutional Neural Network (CNN) for efficient colon cancer detection. In our method, the input histopathological images are normalized before feeding them into our CNN model, and then colon cancer detection is performed. The efficiency of the proposed system is analyzed with publicly available histopathological images database and compared with the state-of-the-art existing methods for colon cancer detection. The result analysis demonstrates that the proposed deep model for colon cancer detection provides a higher accuracy of 99.50%, which is considered the best accuracy compared with the majority of other deep learning approaches. Because of this high result, the proposed approach is computationally efficient.

Work-life balance and teacher engagement in private senior secondary schools in rivers state, nigeria- obasi, k. k. and adieme, f. g.2021

This study examined work-life balance and teacher engagement in private secondary schools in Rivers State. Three research questions and three hypotheses guided the study. The study adopted a correlation research design with a population of 7,143 teachers (2,676 males and 4,467 females) in all the 275 private secondary schools in Rivers State. The sample size of 714 teachers was drawn through a proportionate stratified random sampling technique, which represents 10% of the population. Two self-designed instruments entitled: “Work-life Balance Questionnaire (WLBQ)” and “Teacher Engagement Questionnaire (TEQ)” were used for data collection. The instruments were in two sections –A and B. Section A consisted of the demographic factors while Section B contained the variables that were used to elicit information from the respondents. Section B contained 20 items on Work-life Balance and 15 items on Teacher Engagement respectively. Face and content validities were ensured by experts in the relevant areas. The Cronbach’s Alpha reliability method was used to establish the internal consistency, and indexes of .79 for WLBQ and .83 for TEQ were established respectively. Research questions 1 and 2 were answered using simple regression while research question 3 was answered using multiple regression. Hypotheses 1 and 2 were tested with a t-test associated with simple regression while hypothesis 3 was tested using ANOVA associated with multiple regression at 0.05 alpha level. The findings of the study revealed, among others, that work-life balance significantly and jointly predicted teacher engagement to a very low extent in secondary schools in Rivers State. Based on the findings, it was recommended, among others, that both employers and school managers should create flexible work environments that promote work-life balance for teachers so as to motivate them to perform optimally for the attainment of educational goals.

Analysis of poverty and poverty alleviation measures and information technology: the case of jordan

This study reviews indicators of poverty and the government policies and strategies to reduce and alleviate poverty in Jordan and investigate the effects of information technologies (ITC) on the poverty in Jordan, Population growth rate, GDP at current prices, consumer price index, and human development index in addition to percentage of population use internet, percentage of export and import of ITC of the total trade are used as dependent variables. Data from different resource is collected from 1999 in which the internet started in Jordan till 2018 on the abovementioned variables. The results show that there are negative impacts of the percentage of population using internet on the poverty rate while there are positive effects of human development index and consumer price index on poverty
